feat(pr-review): refuse the first submit_review while reviewed files are unread - #1014
Conversation
…are unread Track per run which reviewed files (source, infra, config, test with a patch) had their diff returned by pr_file_diff, including through the review_files children, which share the parent's handlers. The first submit_review with files unread fails with the list; a second call is recorded. Close-outs and not_applicable verdicts are never held, and a later push only owes the files its kickoff says changed.

Successful tool answers update the tracker, and eligible review submissions are refused once when tracked files remain unread. Follow-up kickoffs and rendered file answers define the files in scope. Suggested reviewers: Merge Risk: 🔵 Low · up to The new check meant to make reviews read every changed file currently backfires. The first time files are unread, the review pass ends and falls back to a partial close-out review instead of letting the reviewer read the files and resubmit. On pushes with more than 60 changed files, the extra files are never checked. Reviews are still recorded, so the impact is reduced review completeness rather than lost data. Both fixes are small and should be made before relying on the check. 🚥 Pre-merge checks | ✅ 5✅ Passed checks (5 passed)

Return unread-file failures to the model.
When the first submit_review call includes unread files, reviewTool uses default failure handling. The MapleToolFailure can end that completion pass before the model receives unreadRefusal. The autonomous runner then performs a close-out that can record only a partial review. Set failureMode: "return" so the model receives the refusal and can read the remaining files before submitting again.

If the changed-file list is cut, return undefined instead of the first 60 paths.
renderFollowUp lists at most MAX_LISTED_CHANGES paths and then adds , and N more. followUpScope removes that suffix and returns only the listed paths. makeReviewCoverage then limits due to those 60 paths. Changed files after the first 60 are never checked on a large push, even though the kickoff tells the reviewer to review them. When the suffix is present, keep every file in scope. After this fix, update the "reads back the listed files when the list is cut" test in packages/backend/src/services/pr-review/findings.test.ts to expect undefined.
