Problem

27% of browser API calls land on a fresh maple-api isolate. Those calls are slower inside the server span, and all of the difference is I/O. The largest single item is OrganizationRegionService.ensureServedHere, which runs in every authenticated request (ApiAuthorizationLayer, ApiAuthorizationV2Layer, SessionAuthorizationLayer).

Production, 2026-10-06 00:00 to 2026-10-08 14:40 UTC:

Region Isolate Requests Server span p50 ensureServedHere p50 / p95 Requests that called Clerk
us first request 842 331 ms 161 / 313 ms 100%
us later requests 2387 167 ms 67 / 193 ms 49%
eu first request 215 443 ms 291 / 514 ms 100%
eu later requests 962 172 ms 0 / 237 ms 43%

Cause

ensureServedHere reads the organization's Clerk public metadata (regions) through getOrganization. The answer was cached only in a per-isolate Map. Isolates that serve browser traffic live about 161 s and serve about 5 requests, so every new isolate paid a full Clerk call (p50 177 ms, p95 704 ms). Warm isolates miss too, once the 60 s (chosen region) or 5 s (region still open) entry expires.

Fix

Region answers now have two tiers:

  1. The isolate memo, as before, now a Ref of a HashMap instead of a mutable Map.
  2. The Workers cache (EdgeCacheService, bucket org-regions), shared by every isolate in a data center.

The shared read is hedged against Clerk. On the preview, a fresh isolate's first Workers cache read took 90 to 340 ms (p10 to p90), against 8 to 13 ms on a warm isolate. That is the same first-I/O cost production Clerk calls show on fresh isolates (+47 ms us, +104 ms eu). With the usual 40 ms read deadline every cold hit was thrown away. With a long deadline, a slow read cost more than Clerk would have. So the shared read runs alone for 40 ms. If it has not answered by then, Clerk is asked as well and the first answer wins. A late hit still beats a slower Clerk call, and a hung read can no longer cost more than Clerk.

EdgeCacheService gains a per-call deadline on rawGetDetailed and a rawDelete, so the service reads, writes and drops its raw-keyed entries without the 40 ms default.

Why the Workers cache:

  • It is already wired into every runtime that builds this service, so there is no new binding or infrastructure.
  • On a warm isolate a read takes 8 to 13 ms (preview p50), against 110 to 180 ms for Clerk.
  • Placement pins the API to one AWS region per instance, so nearly every isolate of an instance shares one data center and one cache.
  • Postgres would add a table, a write path and a Hyperdrive round trip that a fresh isolate pays connection setup for. KV is eventually consistent, slower than the Cache API in our own measurements (CacheBackendLive.ts), and needs a new binding.

TTL and staleness:

  • Each entry stores readAtMs, the time Clerk was read. Both tiers measure age from it, and a stale shared entry is read again from Clerk. A region answer is never reused for longer than today: 60 s once the region is chosen or the choice window has passed, 5 s while the organization can still choose.
  • Data supports 60 s. For 87% (us) and 95% (eu) of first-request-on-isolate calls, another ensureServedHere for the same org in the same region finished within the previous 60 s. Raising the TTL to 15 min adds about one point.
  • Only a yes is stored, as before. A refusal would block an organization that just chose this region. A Clerk failure stores nothing, and the request is still served.
  • OrganizationService.chooseRegion, the only code that writes a region, now calls OrganizationRegionService.forget after the write. That drops the isolate entry and the shared entry.

Spans keep their names. OrganizationRegionService.read now carries cache.hit, cache.layer (isolate, edge or origin) and cache.hedged when Clerk was asked too. The shared read also appears as an EdgeCache.rawGetDetailed child with the usual cache.* attributes.

Tests in OrganizationRegionService.test.ts cover the isolate hit, a fresh isolate hitting the shared tier, the 60 s and 5 s expiry measured from the Clerk read (TestClock), forget after a region write, a Clerk failure not being cached, a refusal not being cached, a hung shared read handing over to Clerk, and a slow shared hit beating a slower Clerk.

Before / After

Metric Before After Change
Fresh isolate, ensureServedHere p50 (preview) 166 ms 112 ms -54 ms (-33%)
Fresh isolate, ensureServedHere p90 (preview) 246 ms 235 ms -11 ms
Fresh isolate, ensureServedHere p95 (preview) 382 ms 286 ms -96 ms
Fresh isolate, answers from the shared tier (preview) 0 of 45 68 of 68
Warm isolate, read misses that reached Clerk (preview) 100% 10 of 43
Warm isolate, shared tier read p50 (preview) n/a 8 ms
Fresh isolate, ensureServedHere p50, us / eu (production) 161 / 291 ms not deployed

Method:

  • Before (production) is the first query under "How to verify", 2026-10-06 00:00 to 2026-10-08 14:40 UTC.
  • Preview numbers come from deployment.environment = 'pr-1317'. A script minted a dev Clerk session and sent waves of 16 parallel GET /v2/ingest_keys requests, 4 s apart, for one organization. That puts some requests on fresh isolates (maple.isolate.request_ordinal = '1') while another isolate has filled the shared tier.
  • Preview Before is the duration of the Clerk getOrganization call on fresh isolates in the first preview run. Before this change that call was all of ensureServedHere. After is ensureServedHere on fresh isolates in the final build (72 requests).
  • Fresh isolates still call Clerk, because their first shared read rarely answers within 40 ms. The saving there is latency, not Clerk volume. Clerk volume drops on warm isolates, where 43 to 49% of production requests called Clerk before.
  • Production After is unmeasured until this deploys. The EU win should be larger than the US one: the cold Clerk call from EU is 285 ms p50, and the shared read stays local.

How to verify after deploy

Run in the internal org with $__orgFilter as a top-level AND. Compare the two days before and after the deploy.

WITH srv AS (
  SELECT TraceId,
    SpanAttributes['maple.isolate.request_ordinal'] = '1' AS cold,
    ResourceAttributes['maple.region'] AS region
  FROM traces
  WHERE $__orgFilter AND $__timeFilter(Timestamp)
AND ServiceName = 'maple-api' AND SpanKind = 'Server'
    AND ResourceAttributes['deployment.environment'] = 'production'
    AND SpanAttributes['maple.isolate.request_ordinal'] != ''
)
SELECT s.region, s.cold, count() AS n,
  round(quantile(0.5)(t.Duration) / 1e6, 1) AS p50_ms,
  round(quantile(0.95)(t.Duration) / 1e6, 1) AS p95_ms
FROM traces t INNER JOIN srv s ON t.TraceId = s.TraceId
WHERE $__orgFilter(t) AND $__timeFilter(t.Timestamp)
  AND t.SpanName = 'OrganizationRegionService.ensureServedHere'
  AND t.ResourceAttributes['deployment.environment'] = 'production'
GROUP BY s.region, s.cold
ORDER BY s.region, s.cold

Where each answer came from:

SELECT ResourceAttributes['maple.region'] AS region,
  SpanAttributes['cache.layer'] AS layer,
  SpanAttributes['cache.hedged'] AS hedged,
  count() AS n,
  round(quantile(0.5)(Duration) / 1e6, 1) AS p50_ms
FROM traces
WHERE $__orgFilter AND $__timeFilter(Timestamp)
  AND SpanName = 'OrganizationRegionService.read'
  AND ResourceAttributes['deployment.environment'] = 'production'
GROUP BY region, layer, hedged
ORDER BY region, n DESC

Expected: on first-request isolates, ensureServedHere p50 drops below 161 ms (us) and 291 ms (eu), most first-request reads report cache.layer = 'edge' with cache.hedged = 'true', and warm-isolate reads that leave the isolate mostly report edge.

ensureServedHere called Clerk on every fresh isolate, about 170 ms of
every cold request. Region answers now go through the Workers cache as
well as the isolate memo, so a new isolate in the same data center
reuses the answer instead of calling Clerk.

Each entry keeps the time Clerk was read and every tier measures age
from it, so the 60 s (chosen) and 5 s (still open) bounds hold end to
end. Only a yes is stored and Clerk failures are never stored.
chooseRegion drops the cached answer after it writes the region.

Maple review

🟢 Confidence 8/10 · likely safe to merge
TTL, refusal and forget semantics line up with EdgeCacheService.getOrCompute; only the untested stale-edge refresh branch is unexercised.
quality 98/100 · 1 note · tests covered · risk medium · 3/3 new units observable

Region answers gain a second tier: reads go through EdgeCacheService bucket org-regions (shared per data center) behind the isolate memo, with the same 60 s/5 s TTLs measured from readAtMs, and chooseRegion now calls forget. The scoping, refusal and failure paths are all preserved, and the new test file pins them; safe to merge.

  • OrganizationRegionService.read reads a shared org-regions edge cache behind its isolate memo
  • makeOrganizationRegionService is exported and takes the directory read, so tests stand in for Clerk
  • OrganizationRegionService.forget drops the isolate and shared entry; chooseRegion calls it
  • OrganizationService.layer now provides OrganizationRegionService.layer

Findings

🔵 Note · F1 · Stale edge entry not rewritten, and its refresh branch has no test

tests · packages/backend/src/services/org/OrganizationRegionService.ts:136-138

The only new branch no test reaches is hit && !isFresh: a shared entry that outlives its TTL because it was written after Clerk answered. The tests cannot reach it — with makeMemoryBackend the write happens at the same TestClock instant as the read, so at readAtMs + ttl the entry is already a miss, not a stale hit. That branch is where the PR's central guarantee ("a region answer is never reused for longer than today") is enforced, so it is the one that most needs pinning: a backend wrapper that stores expiresAt = nowMs + ttl + a lag reproduces it. It also re-reads Clerk without writing the fresh entry back, so every isolate keeps paying the Clerk call until the shared entry's own TTL lapses.

Add a test with a `put` that stores the entry a lag after `readAtMs` and assert the next read comes from the origin with `cache.layer`/`cache.hit = false`; consider `rawPut`-ing the fresh entry in that branch so followers do not repeat the origin read.

📝 Walkthrough

Walkthrough

The organization region service adds isolate-local and shared caching for directory reads. Region updates now clear cached organization data before reading stored metadata back. Tests cover cache reuse, expiration, invalidation, directory failures, and wrong-region responses.

Changes

Organization region service

Layer / File(s) Summary
Raw cache read and delete support
lib/cache/src/edge-cache.ts, packages/backend/src/services/auth/MembershipRevocationService.test.ts, packages/backend/src/services/warehouse/QueryEngineCacheHitRate.test.ts, packages/backend/src/services/warehouse/QueryEngineEvaluateCache.test.ts
rawGetDetailed accepts an optional read-timeout override. The edge-cache API adds rawDelete, and affected test doubles implement or forward that method.
Region reads and cache behavior
packages/backend/src/services/org/OrganizationRegionService.ts, packages/backend/src/services/org/OrganizationRegionService.test.ts
The service checks its isolate-local cache before the shared cache, then reads the directory when neither has a fresh entry. Shared reads have a 1-second deadline and race the directory after 40 ms. Open-region results use a 5-second TTL; chosen-region results use a 60-second TTL. Only answers that serve the current region are cached.
Region update invalidation
packages/backend/src/services/org/OrganizationRegionService.ts, packages/backend/src/services/org/OrganizationRegionService.test.ts, packages/backend/src/services/org/OrganizationService.ts
The region service adds forget. OrganizationService.chooseRegion calls it after writing region metadata and before reading the metadata back. The servesAll layer implements forget as a no-op. Tests cover invalidation of local and shared cached data.

🟡 Confidence 5/10 · needs attention
The only new line since e05f569 is the 1 s read deadline, and the cache semantics it changes are read from lib/cache/src/edge-cache.ts.
quality 88/100 · 1 warning · 1 note · tests partial · risk medium · 1/1 new units observable

Since the last review the only change is a 1 s edge-cache read deadline on the shared org-regions bucket, read on every authenticated request; that deadline is longer than the Clerk call it falls back to, so a queued read stalls the request for up to a second. The two-tier region cache itself is sound and safe to merge once that value is brought back in line.

Findings

🟠 Warning · F2 · 1 s edge-cache deadline stalls the auth path for a 180 ms Clerk read

performance · packages/backend/src/services/org/OrganizationRegionService.ts:121

EdgeCacheService.getOrCompute awaits the backend read for readTimeoutMs before falling back to compute (lib/cache/src/edge-cache.ts:285-291), so a read queued behind the Worker's six connection slots holds every authenticated request — ensureServedHere runs in all three authorization layers — for up to 1 s, and then still pays the ~180 ms Clerk call it could have made immediately. Waiting longer than the compute cannot pay: this is the same Clerk-compute bucket shape for which the repo uses MEMBERSHIP_CACHE_READ_TIMEOUT_MS = 200 (packages/backend/src/services/auth/OrgMembershipService.ts:60), and the abandoned read also keeps its connection slot, which is what makes sibling reads time out (lib/cache/src/edge-cache.ts:159-173).

Use a deadline below the cost of the read it replaces, like the sibling Clerk-compute bucket (`MEMBERSHIP_CACHE_READ_TIMEOUT_MS = 200` in `packages/backend/src/services/auth/OrgMembershipService.ts:60`), or leave the 40 ms default.

135-140: 🚀 Performance & Scalability | 🔵 Trivial | 💤 Low value

Refresh the shared entry after a logically stale hit.

getOrCompute returns a cache hit without running its compute effect. The Workers cache stores the entry with a TTL that starts when the write completes, but readAtMs is captured before the Clerk request. Therefore, while the entry remains in Workers cache but is stale by isFresh, each fresh isolate can call Clerk again. That result updates only the isolate-local remembered map.

The stale window is the Clerk read duration. Math.ceil adds no extra time here because both configured TTLs are exact whole seconds. Use invalidate({ bucket: REGIONS_CACHE_BUCKET, key: orgId }) before recomputing through getOrCompute; this is the supported eviction path and uses the same hashed key. A direct rawPut is not equivalent because it passes the raw key while getOrCompute hashes the key.

The preview showed that a fresh isolate's first Workers cache read takes
90 to 340 ms (p10 to p90), against 13 ms warm. A 40 ms deadline threw
every cold hit away, and a long one let slow reads cost more than Clerk.

The shared read now runs alone for 40 ms. If it has not answered, Clerk
is asked as well and the first answer wins. A late hit still beats a
slower Clerk call, and a hung read can no longer cost more than Clerk.

EdgeCacheService gains a per-call read deadline on rawGetDetailed and a
rawDelete for raw-keyed entries.

Region auth logic changed behind tests for hit, TTL, hedge, forget and Clerk failure; the hedge rests on Effect.race first-success semantics I could not read in effect's source.
quality 100/100 · no findings · tests covered · risk high · 2/2 new units observable

Adds a Workers-cache tier shared across isolates for organization region answers, hedges it against Clerk on cold isolates, and drops both tiers when a region is chosen. Contained to the region-serving auth path and covered by new tests.

  • readShared reads a shared org-regions edge tier, then hedges Clerk against it after 40 ms
  • The shared read moved from getOrCompute to a forked rawGetDetailed; readOrigin writes it with rawPut
  • OrganizationRegionService.forget drops the isolate and shared entries via new EdgeCacheServiceApi.rawDelete
  • chooseRegion calls forget after the write; freshness is measured from readAtMs in every tier

What was checked
  • Staleness: both tiers age entries from readAtMs, TTLs unchanged (OrganizationRegionService.ts:66,77)
  • rawDelete reuses rawPut's raw key, where the previous invalidate hashed it (edge-cache.ts:498,511)
  • Every caller of OrganizationRegionService.layer already provides EdgeCacheServiceLive (apps/api/src/runtime/service-graph.ts:3,146; apps/ai/src/runtime/http-graph.ts:113)

🟢 Confidence 7/10 · likely safe to merge
The hedged race's late-miss branch is the one path that decides between refusing and serving a cross-region request, and it has no test.
quality 90/100 · 1 warning · tests partial · risk medium · 4/4 new units observable

Adds a shared Workers-cache tier (org-regions) under the isolate memo for organization region answers, hedged against Clerk after 40 ms, and invalidates both tiers when a region is chosen. The tiering, TTLs and invalidation hold up; the hedged race's late-miss branch is the one path left unpinned. The two open QueryEngineTimeoutError issues anchored in this file are not addressed — the diff changes how region answers are cached, not the 30 s query ceiling those errors come from.

  • OrganizationRegionService reads region answers from a shared org-regions Workers cache tier
  • readShared races the shared read against Clerk after 40 ms and keeps the first answer
  • EdgeCacheService gains rawDelete and a per-call read deadline on rawGetDetailed
  • OrganizationService.chooseRegion calls forget to drop both tiers after the write

Findings

🟠 Warning · F3 · The late-miss branch of readShared has no test

tests · packages/backend/src/services/org/OrganizationRegionService.ts:172-180

The only test that reaches the hedge hangs the shared read (OrganizationRegionService.test.ts:167), so lateHit never fails, and nothing pins what happens when the shared read resolves to a miss after the 40 ms hedge. That is the case where the read has no entry while Clerk is already in flight, and how it resolves is exactly what decides whether a cross-region request is still refused or is served with no region answer at all.

Add an `it.effect` where the backend `get` is delayed ~100 ms and returns no entry, Clerk answers `chosen("eu")` after ~500 ms, and the shared read is read past the 40 ms hedge; assert `ensureServedHere` fails with `OrganizationWrongRegionError` (so a late miss still hands over to Clerk).

In `packages/backend/src/services/org/OrganizationRegionService.ts:172-180`: The late-miss branch of `readShared` has no test.

Verify the problem exists at that location before changing it, and keep the fix to those lines.
