The RefreshToken grant type is defined in OAuth2GrantType (core/csharp/MicroZen.OAuth2/Definitions/OAuth2GrantType.cs) but has no handler and is not represented in the proto/data GrantType enum.

Done when: refresh-token handling is implemented for Cognito and reflected consistently in the data model and tests.
