plantuml: nest qualified declarations, add rooted form and :: - #501

A qualified name in a nested declaration is appended to the enclosing
scope (like a relative name in C++). A leading `.` or `::` roots it at
the root anchor. The parent / enclosing namespace is derived from the
resulting id.
