[rules score] derive AoUs from safety analysis measures - #510

Split the safety analysis into fault_trees and safety_analysis. A dependable_element now derives its own AoUs from safety-analysis `measures`, so the `assumptions_of_use` attribute is removed. - ScoreReq.AoU extends SafetyMeasure; `root_causes` is mandatory - Forwarding YAML entries require `aou_id@version` - Merge duplicate lobster items (merge_lobster_items) per dependability_analysis and dependable_element, so the same AoU listed by several safety analyses is reported once - Render measure requirements, keep measure deps parse-only; CompReq measures are not fed to lobster-trlc - rst_to_trlc: reject `:root_causes:` on non-AoU directives, skip self-imports of the output package - Default `fta_package` is package and name joined by `__` + `_fta` - Update docs, examples, skills and tests BREAKING: every ScoreReq.AoU needs `root_causes`; the `dependable_element.assumptions_of_use` attribute is gone. Downstream repos must regenerate their sync_skills goldens.
