drafts: true📝 WalkthroughWalkthroughThe change validates Codex rollout records and file identity, rejects legacy writes to paginated history, and adds injection preflight and commit-boundary checks. Refused operations preserve configuration, history, journals, catalogs, and restore evidence. Integration tests cover malformed records, concurrent replacement, native metadata, routing, and restore paths. ChangesPaginated history protection

legacy append remains bound to the validated file identity across the write,
failing closed if the path changes before or after appending; alternatively
coordinate this operation with the native writer. Preserve existing lock
behavior, and add a deterministic race test covering replacement during the
append window, including the non-strict updateSessionMeta path.
- Line 1219: Update every updateSessionMeta catch to rethrow
CodexHistoryIntegrityError instead of suppressing it, while preserving only the
missing-rollout fallback; absent paths should continue returning changed: false.
Add a race test covering a rollout becoming paginated after preflight and
verifying recovery does not update SQLite or commit routing changes.
- Line 239: Update the first-line parsing in the history restoration flow around
the record handling to parse into unknown, catch malformed JSON, and validate
that the result is a non-null object before property checks; convert either
failure into CodexHistoryIntegrityError so syncCodexHistoryProvider and
restoreLegacyOpenaiHistory return their existing integrity result rather than
treating invalid data as legacy history.
In `@src/codex/inject.ts`:
- Line 1260: Introduce a dedicated error type for the commit-boundary refusal
raised by the second historyPreflight() in applyNativeArtifacts(), preserving
compensation and lock rollback behavior. At the injectCodexConfig() boundary,
catch only this error after rollback completes and return { success: false,
message: ... } instead of rejecting; leave unrelated errors propagating. Add
regression coverage for both legacy-uncoordinated and coordinated execution
paths.

Author follow-up at d4ee708: the two inline findings and the outside-diff first-line finding are addressed. The first-line helper accepts the previously validated dev/ino identity, checks the opened descriptor before writes and the current path after writes, and propagates integrity errors. Six deterministic replacement cases cover forward and strict restoration, preserving replacement bytes and restore provenance. Strict partial restoration retains its existing honest The two affected suites passed 151/151; after adding direct coordinator rollback assertions, the three affected restore cases passed again. TypeScript passed on this head. No installed deployment. The previous broad-suite timeout/failures are recorded in the PR body; Draft remains intentional. Earlier malformed @coderabbitai review |

Preserve config when migration occurs during restoration
When Codex adds history_mode after this preflight but while restoreJournalState() or removeCodexConfig() is writing, the config half returns success without revalidating history. The sync path can therefore remove config/profile/journal, and the coordinated async path can commit its remove transition, before the history worker detects history_paginated_requires_native_writer; an active paginated conversation is then left without its referenced provider despite the refusal. Keep the provider artifacts in place until history succeeds, or safely compensate them when the later history operation reports this refusal.

Account for pending backup entries in the preflight
When a migration-capable database has no opencodex rows but still has a nonempty restore manifest—for example, an apply was interrupted after writing its pending manifest but before the database CAS, or a prior restore landed but manifest finalization failed—rows is empty, so this loop never returns the paginated-store refusal. restoreNativeCodex* then changes config/catalog before restoreCodexHistoryProvider() unconditionally rejects the nonempty manifest via assertLegacyHistoryStore(). Include pending manifest entries in this preflight so the restore remains a genuine no-write refusal.
Useful? React with 👍 / 👎.

The containment change from this PR at Closing this superseded PR rather than rebasing the already integrated containment change. #4311 remains open: the newer restore-time migration finding still needs a narrow follow-up against current dev. This closure does not claim universal concurrent-write safety or completed damaged-history recovery. The integration's hosted CI run 34674692660 is still queued at this observation; it is not a passing-test attestation. |

Summary
history_mode, including rows still marked legacy. Never allocate N+1 outside the native writer.Verification
tsc --noEmit: passed, including after integration ofdev@8f1471bca.dev@8f1471bca; the history/injection files tested above are unchanged by that merge.Checklist
This is defensive containment, not native-writer integration. Post-write compensation does not grant a native-writer lock, guarantee every possible concurrent schedule, or make paginated relabeling supported. Keep this PR Draft and #4311 open until the remaining gates are satisfied.
