opencodex decides this by reading Codex own config itself, so the switch does not depend on the
injected URL or on anything a client sends, and turning it off takes effect without a restart.

### Hosted image results in Codex App

When a routed Responses provider returns a completed hosted `image_generation_call`
with base64 image data, opencodex saves the validated image under its local
`artifacts/` directory and delivers a final assistant image message to a locally
connected Codex client. The image stays outside the collapsible progress section.
This applies to streaming and non-streaming Responses, without another generation
request or a change to the selected provider.

When a client replays these generated image messages as assistant history, opencodex
replaces its generated local image links with opaque artifact references before
forwarding that history. This protects the display paths without changing the image
message already shown in the app. It does not redact unrelated user-supplied paths.

This display compatibility requires loopback admission and a recognized Codex client.
Remote and generic API clients retain the provider's hosted response format.
Partial previews and URL-only results are not rendered by this compatibility layer.
Artifacts use the existing retention limit, so save images you want to keep before
older files are pruned. An invalid image or a failed local write produces a visible
failure message instead of a broken image link.

### Built-in image generation (`image_gen`)

Codex's built-in `image_gen` tool does not go through `/v1/responses` — the codex-rs extension

P2 Badge Include the output index in image display state keys

When an upstream reuses the same string item.id for image calls at different output indices, both calls resolve to one state entry: the second output_item.added is suppressed, its completion reuses the first artifact, and the terminal snapshot contains duplicate copies of the first synthetic message instead of the second image. Key indexed events by both output_index and item.id (falling back to the ID alone only when no valid index exists), and add a regression case with duplicate upstream IDs at distinct indices.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Unindexed SSE frames all go to output_index 0.

Line 160 uses 0 as the index whenever event.output_index is missing or invalid. Line 162 still accepts the event when event.item.id is a string. doneEvents then emits output_index: 0 for the synthesized lifecycle frames (Lines 114, 117, 124).

Example: an upstream sends an image output_item.done with a string id and no output_index. The image sits at real position 2. The client receives content_part.added, output_text.delta, and output_item.done frames tagged output_index: 0. Those frames can collide with the message actually at index 0. The state key is still correct because it comes from the id. Only the emitted index is wrong.

The terminal snapshot path does not have this problem because it uses the real array index.

Fix: when hasIndex is false, omit output_index from the synthesized events instead of writing 0.

Proposed fix
-  function doneEvents(item: Row, index: number): Row[] {
+  function doneEvents(item: Row, index: number | undefined): Row[] {
     const state = complete(item, index);
     ...
-    const common = { item_id: message.id, output_index: index, content_index: 0 };
+    const at = index === undefined ? {} : { output_index: index };
+    const common = { item_id: message.id, ...at, content_index: 0 };

Apply the same at spread to the added and done events. Pass hasIndex ? event.output_index : undefined from rewrite. In stateFor, use the key "index:" + index only when index is defined.

Review comment at @src/server/responses-hosted-image-display.ts around lines 152

Update rewrite and doneEvents so synthesized lifecycle events omit output_index
when the upstream frame has no valid output_index, rather than defaulting it to
0. Keep using the item ID to identify state in stateFor when the index is
absent.
