Summary

Closes #7544.

On Windows x64, ocx service install could refuse with CodexUserIdentityRefusal: Windows effective-account lookup timed out. before touching Task Scheduler. The service mutation lock path is keyed on the effective SID, and src/codex/user-identity.ts obtained that SID by starting powershell.exe. On main push run 39812199456 (attempt 1, windows-schtasks) a cold PowerShell start took longer than the existing 30s budget; the rerun passed.

This PR reads the SID in-process instead. The new src/codex/windows-current-sid.ts calls OpenProcessToken → GetTokenInformation(TokenUser) → ConvertSidToStringSidW through bun:ffi, loading advapi32.dll/kernel32.dll from the GetSystemDirectoryW-resolved System32 exactly like the existing LocalAppData path (src/codex/windows-local-appdata.ts). It bounds the UTF-16 read, validates the SID shape, and always calls LocalFree/CloseHandle. No child process starts on x64, so there is no deadline for a cold start to breach.

Refusal semantics are unchanged: where FFI is unavailable (Windows ARM64) the bounded PowerShell helper still runs and its timeout still refuses; when the native API is available and fails, the lookup refuses as Windows effective-account lookup failed. with the OS failure as cause and never falls back to the helper. The 30s budget is untouched.

Decisions

  • Native call instead of whoami /user or a longer budget. whoami is still a child process on the same cold, contended runner, and the lane forbids raising the deadline. The repository already trusts the identical FFI pattern for LocalAppData on x64, so the SID follows it.
  • Process token, not thread token. WindowsIdentity.GetCurrent() prefers an impersonation token; nothing in OpenCodex impersonates, so both report the same user. Recorded in the module header.
  • No new cache for the native path. It costs three syscalls; the existing per-process memo still applies to the PowerShell fallback.
  • Scoped to the failing lookup. withWindowsServiceMutationLock next hardens the lock directory, and the icacls principal is resolved by a separate PowerShell child in src/lib/windows-user-principal.ts that also returns the account name used by Task Scheduler XML. That child is now the first PowerShell start in service install, so the same cold-start condition can still fail there with a different message. Replacing it needs LookupAccountSidW and a name-format parity review in an ACL-sensitive module, so it is filed as [Windows] service install still starts a cold PowerShell for the ACL principal lookup (follow-up to #7544) #7547 instead of being folded in here.
  • Loader failure keeps the helper (declined CodeRabbit suggestion). If bun:ffi cannot load advapi32/kernel32 on x64, the bounded trusted helper runs, as on dev today; its failures still refuse. Once the native API is loaded, an OS failure refuses with no fallback. This matches the LocalAppData contract; the full rationale is in the PR conversation.

Verification

All local runs used an isolated home (HOME, CLAUDE_CONFIG_DIR, OPENCODEX_HOME under mktemp -d) and the pinned Bun 1.4.3 runner via OCX_TEST_RUNNER_BUN; heavy runs were wrapped in /usr/bin/lockf -k /tmp/ocx-heavy-test.lock.

  • Focused: bun scripts/test.ts tests/codex-integration/codex-windows-current-sid.test.ts tests/codex-integration/codex-user-identity.test.ts tests/windows/windows-popup-fix.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts → 49 pass / 2 skip / 0 fail (the skips are the two Windows-x64-only cases).
  • Red → green: with resolveWindowsSid temporarily routed back through the helper only, both [Windows] ocx service install refuses: effective-account SID lookup times out on cold PowerShell start #7544 regression tests fail (2 fail / 2 pass); restored code passes.
  • bun run typecheck → exit 0.
  • bun scripts/test.ts --changed=origin/dev → 36860 pass / 82 skip / 0 fail, 1679 files (exit 0), at the first head; the second head only moves one JSON layout entry, covered by the layout and ratchet runs below
  • bun run privacy:scan → passed. bun run structure:check → passed.
  • bun scripts/test.ts tests/ci-workflows tests/windows (repo-scan tests that read files as data and so sit outside --changed) → 2172 pass / 40 skip / 0 fail, 109 files.
  • File-size ratchet: the first head (5b31d4494) added the test's layout entry to scripts/test-layout/layout.json, taking it from 1999 to 2000 lines, and macos 3/3 failed with NEW_OVERSIZED. The entry now lives in the existing layout-codex-collision.json explicit shard; layout.json is byte-identical to dev, and the ratchet passes locally.
  • Windows-sensitive: yes (new Windows FFI code under src/). Dispatched at exact head 36a0e6a8b:
    • Cross-platform CI lane=all: success, run 39851024844 (all 27 executed jobs green, including windows 1-9/9, keyring windows, macos 1-3/3, macos control, desktop shell). At the first head, dispatch 39839331259 was cancelled from the shared maintainer account mid-queue, and dispatch 39847870684 reported the ratchet failure below, after which I cancelled it
    • Service lifecycle (runs windows-schtasks, the job that failed): success, run 39851029076 (windows-schtasks, linux-systemd, macos-launchd all green; Windows "Install service" step 11s)
    • At the first head 5b31d4494, Service lifecycle 39839389572 was green on all three legs, and the Windows service install step took ~14s, versus ~35s in the failing run. The only change since is the layout shard move.
  • The Windows x64 test compares the native SID with whoami /user /fo csv /nh and with resolveEffectiveUserIdentity(); it runs only on the Windows shards above.
  • A separate full bun run test was not run; the --changed selection above already spans 1679 of the suite's files because user-identity.ts sits under most of the Codex integration graph.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (structure/ops/test-sandbox-cleanup.md owns the Windows runtime namespace contract; no user-facing behavior changes beyond the refusal no longer occurring.)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. (No auth, credential, dependency, Actions, or release-tooling changes. The SID still fails closed and is validated against the SID pattern before use.)

…erShell child

On Windows x64 the service mutation lock resolved the token SID by spawning
powershell.exe, and a cold start on a loaded runner exceeded the 30s budget, so
ocx service install refused (#7544). Read the SID through advapi32 via bun:ffi,
mirroring the LocalAppData known-folder path. PowerShell stays only where FFI is
unavailable (ARM64); native API failures refuse without falling back.

📝 Walkthrough

Windows x64 effective-account SID resolution now uses the process token through native APIs when available. PowerShell remains the fallback when native lookup is unavailable. Native lookup errors produce refusals. Tests cover lookup behavior, validation, resource cleanup, and Windows x64 integration.

Changes: Windows effective-account SID lookup

Layer / File(s) Summary
Read and validate the process-token SID
src/codex/windows-current-sid.ts
The new native lookup loads token APIs from the Windows system directory on x64. It validates token data and the SID string, limits SID reads to 256 code units, and releases acquired resources. The resolver uses PowerShell only when native lookup is unavailable.
Integrate and validate effective identity resolution
src/codex/user-identity.ts, tests/codex-integration/codex-windows-current-sid.test.ts, structure/ops/test-sandbox-cleanup.md, tests/fixtures/test-layout-expected.json, scripts/test-layout/layout-codex-collision.json
Effective identity resolution uses the shared resolver and retains SID validation and normalization. Tests cover fallback, refusal, invalid data, cleanup, and Windows x64 integration. The documentation and test-layout entries include the new lookup.

Full details: Title check

Explanation

The title names the primary change: the effective SID is now read in-process on Windows x64, so service install no longer waits on a cold PowerShell start. The changeset adds windows-current-sid.ts with native token API lookup, keeps PowerShell as a fallback on ARM64, and wires it into user-identity.ts. The title is specific, uses the conventional fix(windows) prefix, and is under 100 characters in its description. It is slightly long at 101 characters, but this does not reduce its relevance.

Full details: Linked Issues check

Explanation

[#7544] Requirement: resolve the current process SID on Windows x64 without depending on PowerShell startup, and fail closed on a genuine failure. Evidence at head 36a0e6a: src/codex/windows-current-sid.ts reads the process token SID in-process through bun:ffi (OpenProcessToken, GetTokenInformation(TokenUser), ConvertSidToStringSidW). It loads advapi32.dll and kernel32.dll from the trusted System32 directory from resolveTrustedWindowsSystemDirectory. It bounds the token buffer (256 bytes) and the SID string (256 code units), validates the SID pattern, and releases the string with LocalFree and the token with CloseHandle in a finally block. src/codex/user-identity.ts routes resolveWindowsSid through resolveWindowsCurrentSid. The PowerShell helper runs only when the native lookup is unavailable (lookup is null, for example on Windows ARM64). Native failures are wrapped as CodexUserIdentityRefusal 'Windows effective-account lookup failed.' with the OS error as cause, and they do not fall back to PowerShell. Helper refusals (timeout, non-zero exit) are rethrown unchanged. The 30s budget is unchanged. Tests named in the prior assessment cover helper bypass, native failure refusal, cleanup, malformed and unterminated SID values, the read bound, and a Windows x64 comparison with whoami. Incremental change since the prior reviewed head 5b31d44 is test-layout metadata only (test entry moved to layout-codex-collision.json); it does not change the requirement evidence. Limitation: the incremental diff blobs were unavailable, so that change was assessed from the PR description and change summary, not from a read diff. Pending CI runs are not coding requirements and were not used in this verdict.

Full details: Out of Scope Changes check

Explanation

All changed areas connect to #7544. src/codex/windows-current-sid.ts implements the native SID read. src/codex/user-identity.ts changes limit to SID routing, refusal mapping, the test-only resolveWindowsSidForTests seam, and comments on the memo and timeout. tests/codex-integration/codex-windows-current-sid.test.ts covers this change. structure/ops/test-sandbox-cleanup.md documents the Windows runtime contract for it. The test-layout fixture and layout-codex-collision.json entries register the new test file. scripts/test-layout/layout.json is unchanged from dev per the PR description. The separate account-principal PowerShell lookup in src/lib/windows-user-principal.ts is not modified and remains deferred to #7547, as the PR description states. No unrelated feature, dependency, auth, ACL, or release change was found in the summarized changes. Limitation: the incremental diff could not be read, so the head-to-head layout change relies on the PR description and summary.

✅ Deterministic PR hygiene checks passed.

layout.json sits one line under the 2000-line file-size ratchet; the entry
added in the previous commit pushed it to 2000 and macos 3/3 failed with
NEW_OVERSIZED. Move the entry to the existing codex-integration explicit
shard so layout.json is unchanged from dev.

🩺 Stability & Availability | 🟠 Major | ⚡ Quick win

Do not downgrade x64 loader failures to the PowerShell fallback.

On Windows x64, loadNativeLookup() catches trusted-directory and DLL-loading failures and sets nativeLookup to null. resolveWindowsCurrentSid() treats null as unsupported FFI and calls powershellLookup(). This reintroduces the PowerShell startup dependency and can restore the bounded timeout that this change is intended to remove. A genuine x64 loader failure must fail closed.

Re CodeRabbit review 5620631867 (outside-diff, Major): "Do not downgrade x64 loader failures to the PowerShell fallback" at src/codex/windows-current-sid.ts loadNativeLookup.

Declining, with these reasons:

  • The fallback is not fail-open. When FFI cannot load, resolveWindowsCurrentSid runs the same bounded helper that dev uses today. Its executable comes from the same GetSystemDirectoryW resolver (resolveTrustedWindowsPowerShellExe → resolveTrustedWindowsSystemDirectory), and its output passes the same SID_PATTERN check. Every helper failure, including the timeout, still raises CodexUserIdentityRefusal. The case where the trusted directory itself cannot be resolved fails the helper's start as well, so it refuses either way.
  • Refusing here would regress availability for no safety gain. A Bun x64 build or host where bun:ffi/dlopen cannot load currently gets a correct SID from PowerShell. With the suggested change, that host would refuse every coordinator, catalog, and service-lock operation outright.
  • It matches the established contract. src/codex/windows-local-appdata.ts makes the same split, documented in structure/ops/test-sandbox-cleanup.md: "Unsupported FFI retains the bounded trusted PowerShell helper; native API failures refuse." The SID module mirrors it on purpose, and the second half is covered here: once the native API is loaded, any OS failure refuses with no helper fallback (each native failure refuses without falling back to the helper… in tests/codex-integration/codex-windows-current-sid.test.ts).

The cold-start exposure this PR removes is the normal x64 path, where loading succeeds; the green Windows shards and windows-schtasks at 36a0e6a8b exercise it.
