fix(ci): bind exception approval labels to the evaluated head - #7699
PR hygiene removed head-specific exception approvals (test-exception-approved, suppression-approved, generated-change-approved, dependency-change-approved, attribution-approved) only when the triggering action was synchronize. pr-hygiene.yml and the enforce gate share the pr-gate-comment-<PR> concurrency group, and GitHub keeps one pending run per group: a later edited/labeled/unlabeled event replaces a queued synchronize run. The run that executes then skips the sweep, and an approval granted for the previous head applies to the new one. The enforce gate re-assessed hygiene with the same live labels.
Approval state is now tied to the evaluated head and reconciled on every run:
pr-hygiene.yml writes one hidden line as the last line of its comment section: the head SHA it evaluated and the count of head_ref_force_pushed/head_ref_restored issue events, read before the PR snapshot (null if that read fails). On every run it keeps those approvals only when the live head equals the recorded head, the rewrite count read after the snapshot is unchanged, and the event is not synchronize. Otherwise it removes them and the comment names what was removed so a maintainer can re-apply after reviewing the head. With no rewrite, fast-forward pushes cannot bring the recorded head back, so an approval applied since the recorded evaluation was applied to this head.
enforce-pr-target.yml applies the same check read-only before its own hygiene re-assessment, so it cannot mark a PR ready on an approval the next hygiene run removes (label removal by GITHUB_TOKEN does not wake the gate). It reads event history only when an approval label is present.
The record is read only from the github-actions[bot] comment whose body starts with GATE_MARKER (its hygiene block), or the one that starts with the hygiene marker while that block does not exist yet. The hygiene upsert and the gate's own comment lookup use the same leading-marker rule, so a marker string echoed inside a comment never reclassifies it. Changed paths echoed by either workflow have control characters escaped (\u000a), and the hygiene block is replaced through a callback, so replacement tokens in a path are written literally; a path cannot add lines to a bot comment. extractHygieneSection also treats a block whose delimiters appear on more than one whole line as absent, for every consumer including the gate rebuild, and the record token must occur exactly once, on the last line; anything else reads as no record, which drops approvals.
Both workflows judge approvals only when the changed-file list was read at the same head the approval check uses (the gate lists files in a separate head-verified snapshot; the hygiene workflow re-reads the PR after listing files). If the head moved in between, no approval is confirmed.
The gate's rebuild keeps the hygiene section through withHygieneSection whenever the rebuilt body has no real delimited block (checked with extractHygieneSection, not a substring). Before, it re-appended the section with a trailing-whitespace replace that did nothing when the rebuilt body had no trailing whitespace, so the section (and now the record) would have been dropped on ordinary gate runs.
Pure logic lives in .github/scripts/pr-hygiene.cjs (parseApprovalRecord, formatApprovalRecord, countHeadRewrites, needsHeadRewriteCount, reconcileHeadBoundApprovals) and pr-quality-messages.cjs (authoritativeHygieneSection, isGateComment, isStandaloneHygieneComment, escapeControlCharacters). No trigger, permission, concurrency or action-pin change; issues.listEvents is a read within the existing token scope.
Behavior to expect after promotion: an open PR that already carries one of these labels has no record yet, so its next hygiene run removes the label once; re-applying it after that run keeps it. structure/ops/docs-and-release.md adds a sentence to the same table row #7698 edited; the merge of dev keeps both sentences.
All on head 7595374509 (based on dev26598df2c4); after merging dev5ab431bec0 (with #7698) into head 43dd17db0d, the focused Bun set (284 pass), node scripts (602 pass), typecheck, privacy:scan and structure:check were re-run and passed, Bun 1.4.3 test runner:
RED before the fix: new tests/ci-workflows/ci-pr-hygiene-approvals.test.ts (runs the real pr-hygiene.yml and enforce-pr-target.yml scripts against a recording client) → 4 pass / 13 fail, exit 1; the first failure is an edited run after a head change that kept the old-head approval.
GREEN at this head: 26 pass / 0 fail. Covers: edited run after head change, approval at unchanged head kept and recorded, synchronize, rewrite since the record, revoke/push/re-approve/rewrite-back, missing record, record last on pass and fail paths, a crafted path with newline and delimiter text, a standalone hygiene comment echoing the gate marker through standalone → gate run → next hygiene run, pre-snapshot count, unreadable history (before/after/both), paginated events, removal 404 vs other errors (no record published), the accepted event-visibility limitation, four gate cases, a gate rebuild followed by a same-head hygiene run, four replacement-token path cases, a head move between the PR snapshot and the file listing (hygiene, and a maintainer gate case), and an echoed path containing the block marker text not suppressing the gate's preservation. Review-driven cases fail on the earlier commits of this PR (16 pass / 2 fail on the first commit, 18 pass / 5 fail on the second, 23 pass / 2 fail on the third, 25 pass / 1 fail on the fourth).
node --test .github/scripts/*.test.cjs → 602 pass / 0 fail (new parse/format/reconcile, block-selection, escaping, classification and literal-replacement cases; the enforce-pr-target.test.cjs text pin now expects comments.find(isGateComment)).
bun test of the new file + ci-workflows.test.ts, zz-pr-coderabbit-readiness-revalidation.test.ts, pr-readiness-reattest.test.ts, test-layout*.test.ts, file-size-ratchet.test.ts, structure-ssot.test.ts → 284 pass / 0 fail (exit 0).
bun run typecheck → exit 0. OCX_TEST_RUNNER_BUN=<bun 1.4.3> bun run test:changed → 26 pass / 0 fail (the workflows and .cjs scripts are outside the import graph, hence the explicit files above).
bun run privacy:scan → passed. bun run structure:check → passed.
NOT RUN: full bun run test — shared host with concurrent lanes; one earlier combined run hit a load-related 5 s timeout in file-size-ratchet that passed alone (9/9) and in later combined runs. Left to CI.
Known limitation: a rewrite not yet visible in issues.listEvents at either read is not counted, and its later visibility does not start a run by itself; a test pins this behavior.
pull_request_target workflows load from the default branch, so live behavior changes after promotion to main.

PR hygiene removed head-specific exception approvals (test-exception-,
suppression-, generated-change-, dependency-change-, attribution-approved)
only when the triggering action was synchronize. The hygiene and gate
workflows share one concurrency group, and a queued synchronize run can be
replaced by a later event before it executes, so approvals granted for an
earlier head could stay in effect for a new one.
The hygiene workflow now records, as the last line of its comment section,
the head it evaluated and the number of head-rewrite issue events read
before that snapshot. On every run it keeps those approvals only when the
live head equals the recorded head, the rewrite count is unchanged, and the
event is not a synchronize; otherwise it removes them and says so in the
comment. The enforce gate applies the same check read-only before its own
hygiene re-assessment.
The record is read only from the workflow bot's gate comment block (or the
standalone hygiene comment before that block exists). A block whose
delimiters appear on more than one line is treated as absent, including by
the gate rebuild, so echoed file paths cannot move the block boundary.

Walkthrough
The PR binds five approval labels to a recorded head SHA and rewrite count. The hygiene workflow reconciles these labels and writes the record to comments. The enforcement gate reads the record and excludes stale approvals before evaluating hygiene failures.

Approval record and comment selection .github/scripts/pr-hygiene.cjs, .github/scripts/pr-quality-messages.cjs, .github/scripts/pr-hygiene.test.cjs, .github/scripts/pr-quality-messages.test.cjs, .github/scripts/enforce-pr-target.test.cjs
Approval records require a valid full SHA and rewrite count, appear once as the final nonblank record line, and contain only expected keys. Comment helpers identify gate and standalone hygiene comments and select the authoritative section. Tests cover record validation, comment selection, control-character escaping, and literal replacement.
Hygiene workflow reconciliation and record writing .github/workflows/pr-hygiene.yml, tests/ci-workflows/ci-pr-hygiene-approvals.test.ts, structure/ops/docs-and-release.md, scripts/test-layout/layout-clients.json, tests/fixtures/test-layout-expected.json
The workflow reconciles approval labels against the current head and rewrite history on each run. It removes stale labels and records the head and rewrite count in passing and failing comments. Tests cover reconciliation, event-history reads, and comment updates. The workflow description and test-layout mapping are updated.
Enforcement gate and workflow validation .github/workflows/enforce-pr-target.yml, tests/ci-workflows/ci-pr-hygiene-approvals.test.ts
The enforcement gate reads the recorded hygiene section and excludes stale approvals from hygiene evaluation. Tests cover matching and stale records, rewrite-history changes or read failures, and cases with no approval label.

Explanation
The title "fix(ci): bind exception approval labels to the evaluated head" matches the main change in the changeset. The raw summary and PR objectives describe head-bound approval tracking for the five exception labels in .github/workflows/pr-hygiene.yml, the parsing and reconciliation helpers in .github/scripts/pr-hygiene.cjs, and the read-only check in .github/workflows/enforce-pr-target.yml. The title uses the conventional fix(ci) prefix, is one sentence, and avoids file lists and vague terms. It does not mention the secondary hygiene-comment selection and control-character escaping changes, which is acceptable because the title does not need to cover every detail.
Full details: Linked Issues check
Explanation
Issue #7652 requires head-specific approval labels to be removed when a run evaluates a different or unconfirmed head, without relying on the triggering action. .github/workflows/pr-hygiene.yml now reads the recorded approval state on every run, verifies the file snapshot against the PR head, counts rewrite events when required, and calls reconcileHeadBoundApprovals for the five approval labels. It writes the evaluated head and rewrite count on both pass and fail paths. .github/workflows/enforce-pr-target.yml applies the same read-only check before hygiene evaluation. tests/ci-workflows/ci-pr-hygiene-approvals.test.ts covers edited runs after head changes, synchronize runs, missing records, rewrite changes, unreadable history, and gate behavior. The reported residual event-visibility limitation is identified and does not leave a concrete requirement from #7652 unmet.
Full details: Out of Scope Changes check
Explanation
The change to scripts/test-layout/layout-clients.json adds the link-parity-reporter.test.ts entry to the clients layout. This test is unrelated to issue #7652 and is not required by the head-bound approval implementation. The ci-pr-hygiene-approvals.test.ts layout entry and its fixture update are in scope because they register the new approval tests. The other workflow, helper, test, and documentation changes support #7652.
Resolution
Remove the link-parity-reporter.test.ts entry from scripts/test-layout/layout-clients.json. Keep the ci-pr-hygiene-approvals.test.ts entry. Move the link-parity layout change to a separate PR with its own linked requirement.

Review of the head-bound approval record found that a standalone hygiene
comment echoing a changed path containing the gate marker and a delimiter
pair was read as the gate comment, so a record from the path could stand in
for the one the workflow wrote.
- A comment is the gate comment only when its body starts with GATE_MARKER,
and the standalone hygiene comment only when it starts with HYGIENE_MARKER.
The record selector, the hygiene workflow's upsert and the gate's own
comment lookup use the same rule.
- Changed paths echoed by both workflows have control characters escaped, so
a path can no longer add lines to a bot comment.
Adds the standalone -> gate -> next-run regression and unit cases.
…t literally
- The gate re-appended the preserved hygiene section with
body.replace(/\s+$/, ...), which does nothing when the rebuilt body has no
trailing whitespace, so a normal gate run dropped the section and with it
the approval record. It now goes through withHygieneSection, which appends
unconditionally.
- withHygieneSection replaces an existing block through a callback, so
replacement tokens such as $& in an echoed path are written literally
instead of re-inserting the previous block.
Adds a gate-rebuild -> hygiene regression and replacement-token cases.
Bind approval reconciliation to the verified file-snapshot head.
The file snapshot compares only its two internal reads. It does not compare the verified head with the initial pr.head.sha. A push can therefore produce a new file list while approvalInput.headSha still identifies the old head. An unchanged rewrite count can retain old approval labels and waive hygiene failures for the new files.
A remaining failure correctly enters mustDraft and returns before the ready path. However, stale approvals can suppress the last hygiene failure, leaving failures.length === 0. The final head check then protects contributor PRs, but not the maintainer no-checklist path.
If the verified snapshot head differs from the initial PR head, refetch and reassess from a consistent head, or fail closed by treating all head-bound approvals as stale. Apply this before hygiene evaluation, regardless of failures.length or checklist type. Add a maintainer regression case with an old initial head, a new file-snapshot head, and stale approval labels; assert that readiness state is not cleared.

CodeRabbit review: the gate checked head-bound approvals against its first
PR snapshot, but lists changed files in a separate head-verified snapshot.
A push in between let approvals bound to the earlier head waive hygiene
failures in the later head's files (on the maintainer path the final head
check does not cover that). The hygiene workflow had the same gap between
pulls.get and listFiles.
Both now keep head-bound approvals only when the changed-file list was read
at the same head the approval check uses; otherwise none is confirmed.
Adds a hygiene case and a maintainer gate case with a head move between the
snapshots.

Match the actual hygiene block, not a substring.
body.includes("pr-hygiene-block") also matches arbitrary text, including a changed file path such as docs/pr-hygiene-block.md. In that case, this branch skips withHygieneSection() and the next update drops the existing hygiene section and approval record. Use extractHygieneSection(body) === null or another delimiter-aware check.

CodeRabbit review: the gate skipped re-appending the preserved hygiene
section whenever the rebuilt body contained the text "pr-hygiene-block"
anywhere, so an echoed changed path such as src/pr-hygiene-block.ts dropped
the section and its approval record. Check for an actual delimited block

This pull request was already a draft. Its draft status will be preserved after every issue above is resolved.
Integration evidence (owner lane M1, maintainer integration): independent gpt-6.1-sol review PASS (rounds 4-6) at 7595374; the current head 43dd17d only adds a merge of origin/dev that resolves the shared row in structure/ops/docs-and-release.md after #7698. Exact-head PR CI run 40144074710 and exact-head ci.yml lane=all run 40144071540 both succeeded. Note for maintainers: after this reaches main, an open PR that already carries one of the exception approval labels loses it once on its next hygiene run (no head record yet); re-applying keeps it.

Merged by the triage manager as maintainer integration (MAINTAINERS.md, dev only) with green exact-head PR CI, an independent review PASS, and the owner's approval of this bundle.

Result for requested head 7595374509: PASS. Full macOS coverage. No INTRODUCED failures. Two control-leg tests failed once on child-process timeouts; neither reproduces on this head or on dev (details below).
Head moved: after this run started, the PR head moved to 43dd17db0d6823ad7369ba4731eacf2d930c0c54 (a merge of dev 5ab431bec0 into the branch), and the PR merged at that head. This report covers 7595374509. A separate run at 43dd17db0d is queued and will be posted as its own comment.
Host: Apple Mac16,9, macOS 26.6.2 (25G83), arm64, 16 cores, 64 GB. Command Line Tools only (no Xcode.app), Swift 6.4, rustc/cargo 1.99.0.
Exact head:7595374509dda7e8a7324cccb7c5501f23154476 (the requested head; merge base dev 26598df2c4). Every leg used a fresh detached scratch worktree.
Bun: official bun-darwin-aarch64 release binaries pinned by this head: test runner testRunnerBun 1.4.3 (c6da4a4d3) for the four test legs, runtime dependencies.bun 1.4.4 (38afee6c8) for the widget leg.
Env: clean env -i (HOME/USER/TMPDIR/LANG/PATH only), CI=true RUNNER_OS=macOS, GNU coreutils timeout on PATH (as the workflow's "Setup bounded batch utilities" step adds), OPENCODEX_TEST_TRUSTED_FIXTURE_DIR set to a private 0700 scratch directory per leg, GIT_DIR/GIT_INDEX_FILE/GIT_WORK_TREE unset. Test legs used the workflow env: BUN_TEST_FILE_SCOPE=all BUN_TEST_BATCH_SIZE=12 BUN_TEST_PARALLEL=1 BUN_TEST_BATCH_TIMEOUT_SECONDS=300 OCX_TEST_NO_QUEUE=1 OCX_TEST_FULL_SUITE=1.
Coverage:scripts/ci/run-bun-test-batches.sh ran from a copy whose only change turns the two in-loop exit "$status" lines into "record and continue" (plus an absolute scripts/ci path for its sourced helpers). Every batch ran, and the copy exits with the first failing status. Legs ran one at a time under a host lock after a pressure check.
Selector: this diff matches none of the native, setup_action, or remote_helper filters, so the macOS keyring, npm-global, setup-action, and remote-helper legs are not selected. macos widget + bundle runs on every non-PR event and is included.

The shard legs cover 869 + 869 + 870 = 2608 files, which equals the control's 2608-file membership.
Failures and classification
macos control batch 105/254 (12 files under tests/codex-integration/), two child-process timeouts:
tests/codex-integration/native-profile-store.test.ts: native-profile recovery journal storage > rejects a vault replaced by a FIFO after layout validation (vault FIFO replacement probe did not terminate cleanly, spawnSync ... ETIMEDOUT, :445)
tests/codex-integration/native-ultrafast-picker.test.ts: convergence writes native Ultrafast into the catalog and models cache, and removes it when disabled (spawnSync child returned status null after its 30 s timeout, :187)
Classification: NOT REPRODUCED (timing flake), not INTRODUCED and not BASE-IDENTICAL. On dev 26598df2c4 with the same runtime and env, both files pass alone (16/0 and 12/0) and the exact 12-file batch passes (247 pass/1 skip/0 fail). On this head both files passed in their shard legs, pass alone (16/0 and 12/0), and the exact 12-file batch rerun passes (247/1/0). This PR changes only .github/scripts, .github/workflows, a structure doc, the test-layout maps, and tests/ci-workflows/ci-pr-hygiene-approvals.test.ts; it touches no runtime code. Host load average was about 9 to 10 from unrelated processes.
macos widget + bundle: Test MenuBarCore exit 1 (MenuBarCoreTests 123 passed, 0 failed; NativeTrayTests failed to compile: external macro implementation type 'SwiftUIMacros.StateMacro' could not be found, app/Sources/NativeTray/AccountSwitch.swift:32). Build unsigned desktop app exit 1 from the same NativeTray Swift compile in build.rs. Verify WidgetKit appex and desktop app exit 1 because no app bundle was produced. Classification: BASE-IDENTICAL. dev 26598df2c4 fails the same three steps with the same SwiftUIMacros root cause. This host has Command Line Tools without Xcode, so the SwiftUI macro plugin is unavailable, and this PR does not touch app/ or desktop/. Build WidgetKit appex passed.
NOT RUN: macOS keyring, npm-global, setup-action, and remote-helper legs (not selected for this diff). A signed or Xcode-backed widget build is not possible on this host.

Summary
PR hygiene removed head-specific exception approvals (
test-exception-approved,suppression-approved,generated-change-approved,dependency-change-approved,attribution-approved) only when the triggering action wassynchronize.pr-hygiene.ymland the enforce gate share thepr-gate-comment-<PR>concurrency group, and GitHub keeps one pending run per group: a lateredited/labeled/unlabeledevent replaces a queuedsynchronizerun. The run that executes then skips the sweep, and an approval granted for the previous head applies to the new one. The enforce gate re-assessed hygiene with the same live labels.Approval state is now tied to the evaluated head and reconciled on every run:
pr-hygiene.ymlwrites one hidden line as the last line of its comment section: the head SHA it evaluated and the count ofhead_ref_force_pushed/head_ref_restoredissue events, read before the PR snapshot (nullif that read fails). On every run it keeps those approvals only when the live head equals the recorded head, the rewrite count read after the snapshot is unchanged, and the event is notsynchronize. Otherwise it removes them and the comment names what was removed so a maintainer can re-apply after reviewing the head. With no rewrite, fast-forward pushes cannot bring the recorded head back, so an approval applied since the recorded evaluation was applied to this head.enforce-pr-target.ymlapplies the same check read-only before its own hygiene re-assessment, so it cannot mark a PR ready on an approval the next hygiene run removes (label removal byGITHUB_TOKENdoes not wake the gate). It reads event history only when an approval label is present.github-actions[bot]comment whose body starts withGATE_MARKER(its hygiene block), or the one that starts with the hygiene marker while that block does not exist yet. The hygiene upsert and the gate's own comment lookup use the same leading-marker rule, so a marker string echoed inside a comment never reclassifies it. Changed paths echoed by either workflow have control characters escaped (\u000a), and the hygiene block is replaced through a callback, so replacement tokens in a path are written literally; a path cannot add lines to a bot comment.extractHygieneSectionalso treats a block whose delimiters appear on more than one whole line as absent, for every consumer including the gate rebuild, and the record token must occur exactly once, on the last line; anything else reads as no record, which drops approvals.withHygieneSectionwhenever the rebuilt body has no real delimited block (checked withextractHygieneSection, not a substring). Before, it re-appended the section with a trailing-whitespace replace that did nothing when the rebuilt body had no trailing whitespace, so the section (and now the record) would have been dropped on ordinary gate runs..github/scripts/pr-hygiene.cjs(parseApprovalRecord,formatApprovalRecord,countHeadRewrites,needsHeadRewriteCount,reconcileHeadBoundApprovals) andpr-quality-messages.cjs(authoritativeHygieneSection,isGateComment,isStandaloneHygieneComment,escapeControlCharacters). No trigger, permission, concurrency or action-pin change;issues.listEventsis a read within the existing token scope.Behavior to expect after promotion: an open PR that already carries one of these labels has no record yet, so its next hygiene run removes the label once; re-applying it after that run keeps it.
