fix(service): treat headless GUI-domain launchctl 125 as an unreachable domain (#7697) - #7745
Carries #7697's fix from #7710 (by @chenyb999-zhcn), which targeted main, onto dev. On a headless Mac, launchctl print gui/<uid>/<label> exits 125 ("Domain does not support specified action") because no GUI session has booted that domain. The service-manager probe treated that exit code as "could not ask", so ownership came back unknown and every admission-gated Codex write was refused, even on a machine with nothing installed.
A 125 from a gui/ domain now counts like 112: a label-independent answer about the domain, which cannot be holding our job. The probe then continues to the user domain and the on-disk plist checks. A 125 from the user/<uid> fallback still fails closed as unknown.
src/service-manager-probe.ts: the original commit, cherry-picked unchanged and with its authorship kept.
tests/codex-integration/codex-service-manager-probe.test.ts: the author's four regressions (GUI 125 + user 113 with and without a staged plist, GUI 125 + a loaded user domain, and user-domain 125 still unknown).
structure/ops/service-and-sidecars.md: records the rule in the owning doc. The doc sits at its line budget, so the rule is one inline clause.

Summary by CodeRabbit
Bug Fixes
Improved macOS service detection when a GUI-domain query returns exit status 125. The probe checks the console-session roster and continues checking the user domain before determining whether a service is present, absent, or unknown. It reports a GUI domain as unreachable only when complete, valid roster data confirms that the UID has no GUI session; inconclusive roster data or a user-domain exit status of 125 leaves the result unknown.
Documentation
Clarified the evidence required to determine that a GUI domain is unreachable.
…omain answer
On a headless macOS session launchctl print gui/<uid>/<label> exits 125
("Domain does not support specified action") before ever looking at the
label. The probe returned unknown at that point, skipping the user-domain
query and the plist checks, so absence could never be proven and every
admission-gated write was fenced on a machine with nothing installed.
125 from the gui/ domain is now counted like 112 - a label-independent
answer about the domain - and the probe continues to the user domain and
the on-disk definition checks. A user-domain 125 stays unknown: the
fallback must be able to answer, so its failure is a failure to ask.

Walkthrough
The launchd probe now checks bounded ioreg and plutil session-roster evidence before treating GUI-domain status 125 as unreachable. It then checks the user domain and staged plist. Invalid or unavailable roster evidence, and user-domain status 125, produce an unknown result.
ProbeRunner accepts optional stdin, which defaultProbeRunner forwards to spawnSync. Bounded ioreg and plutil calls confirm GUI-session absence only when the roster data is complete and valid.
Domain classification and regression coverage src/service-manager-probe.ts, tests/codex-integration/codex-service-manager-probe.test.ts, structure/ops/service-and-sidecars.md
When the GUI-domain query returns status 125, the probe continues only if the roster excludes the queried UID. It then checks the user domain and staged plist. Tests cover present, absent, and unknown outcomes, including invalid or unavailable roster data. The documentation describes the corroboration requirement.

This change lets the macOS service probe continue past a GUI-domain failure on headless machines. It relies on an inference that has not yet been checked on real hardware: that a missing console session means no GUI job is loaded. If that is wrong, the probe could report a running service as absent. The maintainers are already holding the PR for that evidence, so confirm the headless, loaded-job, and Screen Sharing-only cases before merging.

Full details: Title check
Explanation
The title names the primary change: GUI-domain launchctl exit 125 on headless macOS is now treated as an unreachable domain, which lets the probe continue to the user domain and plist checks. The changeset matches this. The raw summary says a corroborated GUI status 125 counts as an unreachable domain, and the objectives say the requested behavior is to continue checks rather than stop. The title also carries a conventional-commit prefix and an issue number, which is acceptable. The title is slightly overstated, though. The objectives say the PR is held and the GUI-125 inference is not yet verified natively, and the change now depends on ioreg/plutil corroboration. The title does not mention that gating. This is a minor gap, not a mismatch.
Full details: Out of Scope Changes check
Explanation
The whole-PR diff from merge base e3ebd82a5b51926d964e5d0e86dd0177aed9e2da to head aac83fdee9ae02f4d9d822ca9740a94aa9e679f0 changes three areas. All three serve the GUI-domain 125 handling in #7697.
src/service-manager-probe.ts: ProbeRunner gains an optional input option, and defaultProbeRunner forwards it to spawnSync. The constant LAUNCHCTL_DOMAIN_UNSUPPORTED_ACTION (125) is added. The function launchdGuiSessionAbsent (the ioreg and plutil roster read) is added. The GUI 125 branch in inspectLaunchd is added. The absent-result comment is reworded.
structure/ops/service-and-sidecars.md: a one-clause note records the rule and cites #7697.
tests/codex-integration/codex-service-manager-probe.test.ts: regression tests for GUI 125, user-domain 113 and 125, and roster outcomes.
No unrelated runtime behavior, refactoring, or cleanup appears in the diff. The roster read is an added safety check inside the #7697 branch. Whether that check matches the issue's requirement is assessed under linkedIssuesAssessment, not here.
The PR description is stale. It says the probe change is the original commit "cherry-picked unchanged." It does not mention the ioreg and plutil roster read or the runner input option. This affects documentation accuracy, not scope.
Full details: Linked Issues check
Explanation
[#7697] Requirement: a GUI-domain launchctl print exit 125 must not stop the service-manager probe. The probe must continue to the user-domain query and the plist checks. The issue gives an isolated reproduction (GUI-domain query returns 125, every other runner reply returns 113, empty temporary home) and expects { kind: "absent" }.
Code at the reviewed head (src/service-manager-probe.ts):
In inspectLaunchd, the GUI 125 branch (printed.status === LAUNCHCTL_DOMAIN_UNSUPPORTED_ACTION && domain.startsWith("gui/")) calls launchdGuiSessionAbsent(deps.run, deps.uid) first. When that returns false, the probe returns unknown("GUI login session absence could not be established").
launchdGuiSessionAbsent first runs /usr/sbin/ioreg -a -r -d 1 -n Root. In the issue's reproduction, the runner's args[1] is -a, so the gui/ prefix test is false and the runner returns status 113 with empty stdout. The check registry.status !== 0 || !registry.stdout.trim() then returns false.
Result: the issue's own reproduction yields unknown, not absent. The expected outcome is not produced.
The new tests in tests/codex-integration/codex-service-manager-probe.test.ts mock a valid empty IOConsoleUsers roster for the ioreg and plutil calls. They do not run the issue's literal reproduction, so no test covers that case.
Still supported at head: a user-domain 125 remains unknown. Timeouts and spawn failures remain unknown. Plist checks run after a corroborated GUI 125.
The PR objectives describe the roster gate as a maintainer safety refinement after native review. #7697 does not request that gate. The current issue text controls the requirement. The safety question (roster coverage of every GUI context, synchronization with launchd domain creation and teardown, Fast User Switching, and the absence of native headless evidence) is open. That open question is a maintainer decision. It does not change the issue's stated expected result.
Resolution
A maintainer must choose one path. Then align the issue, the code, and the tests.
(1) Keep the roster gate. Amend #7697 so its expected behavior and reproduction include a valid empty IOConsoleUsers reply for uid 501 from both /usr/sbin/ioreg and /usr/bin/plutil. Add a regression test in tests/codex-integration/codex-service-manager-probe.test.ts that uses the issue's literal runner shape with that roster reply and expects { kind: "absent" }. Keep the existing unknown tests for malformed, unreadable, timed-out, and other-UID roster replies. Record the native macOS evidence in the structure/ops/service-and-sidecars.md note before merge.
(2) Drop the roster gate. In inspectLaunchd, remove the launchdGuiSessionAbsent call from the GUI 125 branch so that the branch increments unreachableDomains directly, as #7697 requests. Remove launchdGuiSessionAbsent and the roster-specific tests. Remove the options.input parameter from ProbeRunner and defaultProbeRunner if no other code uses it. Keep the user-domain 125 unknown test.
Either path needs a maintainer decision on the safety question before this PR can pass.

Maintainer hold: DEFERRED_POST_RELEASE (not merging for v2.93.0).
An independent review of head 376f4a553 found a blocking evidence gap. Locally the change is clean: codex-service-manager-probe, -hardening and systemd-env-probe pass 178 / 0, typecheck and structure:check are clean.
Blocking: a gui/ 125 is not yet shown to prove absence (src/service-manager-probe.ts:379). [Bug]: Headless GUI launchctl exit 125 prevents service-manager absence proof #7697 records one headless observation (GUI 125, user 113). It does not show that 125 can never come back while a GUI-domain job is still loaded. The gui/ and user/ domains have separate service sets, so a user-domain 113 cannot rule out a GUI job either. The current matrix with no plist or state on disk is: GUI 125 / user 113 → absent → owned; GUI 125 / user 112 → absent → owned. If an unsupported inspection can coexist with a loaded GUI job whose plist was removed, ownership preflight would admit writes it should refuse. This is an unverified gap, not a reproduced bypass.
Non-blocking: the updated comment at :396 calls 113 label-independent. 113 is service-scoped; only 112 (and possibly 125) are about the domain.
To land after the release, we need native macOS evidence for: (a) a logged-in GUI session with the opencodex job loaded, launchctl print gui/<uid>/<label> run both locally and from an SSH session into the same machine (does it ever exit 125?); (b) the same after deleting the plist while the job stays loaded; (c) the headless case from #7697. If 125 only ever occurs when no GUI login session exists, the rule can stay, with that evidence cited in the structure note. Otherwise, a GUI 125 should stay unknown unless something else establishes absence.
Require a bounded read of the complete IOConsoleUsers UID roster before
treating GUI-domain 125 as unreachable. Keep matching, malformed, unreadable
and timed-out evidence unknown, including switched-out GUI sessions.
Preserve the independent user-domain and plist checks, and clarify that
113 is service-scoped. Cover the probe and downstream write admission with
Windows-portable injected runners; sync the owning structure clause.
Release class: POST-RELEASE. This lands after v2.93.0 is published on main, and won't merge before then.
I pushed a maintainer update to the branch (54be78902). It merges current dev (no rebase or force-push) and replaces the bare GUI-125 rule with a conservative one. A gui/ 125 now counts as unreachable only when bounded ioreg/plutil evidence shows the uid has no console session in IOConsoleUsers. Fast User Switching keeps other GUI sessions alive, so console ownership alone isn't enough. Matching sessions, malformed evidence, read failures and timeouts all stay unknown, a user/ 125 stays unknown, and the 113 comment now calls it service-scoped. I still couldn't find a primary source showing that 125 alone means there is no GUI session, so native macOS outputs from a headless and a logged-in session would still help; @chenyb999-zhcn, if you can run them, please post them here. An independent review of this head is running now, and exact-head CI runs closer to the merge.

Independent round-2 review of 54be78902: MUSTFIX, 1 blocking finding. Everything else in the new probe passed review: fixed executable paths and argv, plist passed through stdin, 2-second bounds, conservative parsing, failure and timeout handling, switched-out sessions, user/ 125, unknown propagating downstream, the 113 comment, the structure clause and the size caps.
The remaining blocker: IOConsoleUsers excluding the uid is treated as proof that no gui/<uid> domain can hold the job (src/service-manager-probe.ts:367–371, :403 → ownership-preflight.ts:237owned). Apple's XNU sources publish that roster and TN2083 describes session contexts, but neither guarantees that the roster covers every GUI context (remote or Screen Sharing sessions) or that it is synchronized with launchd creating and tearing down the domain. Until that's shown, a GUI 125 must stay unknown. This PR stays held and is POST-RELEASE in any case.
It can land once there is native evidence. @chenyb999-zhcn, if you can run these on a Mac, please paste the outputs here:
The same commands over SSH while the same user is logged in at the console with the opencodex service loaded (launchctl print gui/$(id -u)/<label> should exit 0 there).
The same while logged in only through Screen Sharing/ARD, if possible.
If case 2 or 3 ever yields 125 while the job is loaded, or a roster that omits the uid, the rule stays unknown. Otherwise the outputs become the evidence cited in structure/ops/service-and-sidecars.md.
Post-release status (v2.93.0 is published on main): this PR stays held. The one remaining blocker is the evidence gap from round 2. Nothing in this repository can close it, and this machine has no macOS host to produce it.
Merging needs native macOS output for the three cases in the comment above:
Headless over SSH only.
SSH while the same user is logged in at the console with the opencodex job loaded.
Screen Sharing or ARD, if possible.
Each case needs the launchctl print gui/<uid> and gui/<uid>/<label> exit codes plus the IOConsoleUsers roster.
If those outputs show that a GUI-domain 125 never coexists with a loaded job and that the roster tracks every GUI session, the current head (54be78902) can be refreshed onto dev, re-reviewed and landed. If they show coexistence, a GUI 125 has to stay unknown, and #7697 needs a different approach.
@chenyb999-zhcn, if you can run them, please paste the outputs here.

Roster returned by that pipeline (session UUID redacted):
With a console session for the uid, SSH into the same uid sees gui/<uid> (exit 0), not 125. So 125 is not an artifact of asking from SSH while that GUI session exists.
The PR's roster pipeline works on macOS 27.0 and lists the logged-in uid as on console. In this state the PR's corroboration correctly withholds the GUI-125 relaxation, although no 125 occurred here to exercise it.
Converting the whole ioreg -a document with plutil -convert json fails on this host (Invalid object in plist for JSON format). The PR's -extract 0.IOConsoleUsers form avoids that; keep it if the code changes.
Still missing before the GUI-125 rule can land (unchanged blockers):
(a) headless: no console login for the uid, to see whether gui/<uid> answers 125 and the roster omits the uid. Not observed; this host was logged in.
(b) the opencodex job loaded in gui/<uid> while queried over SSH. Not observed: the job is not installed here, and installing it was out of scope for a read-only check.
(c) Screen Sharing / ARD-only sessions, and whether they appear in IOConsoleUsers. Not observed.
…contract
ioreg now returns a plist holding 0.IOConsoleUsers, and plutil answers only the exact extraction of that input. Mutating the production extraction key turns four positive cases red.
