fix(history): bound the integrity check wait (#7984) - #7997
Async request-history readers (dashboard history, analytics) wait on the background PRAGMA quick_check that #7972 moved to a worker. A worker that stayed alive but never answered left inspect() pending forever, and every async reader waiting on the shared open path waited with it.
The integrity check now has a 30 s bound (REQUEST_HISTORY_INTEGRITY_CHECK_TIMEOUT_MS, the same scale as the existing 30 s failure cooldown). When it elapses, the monitor:
records an operational failure, so the existing cooldown applies and a later inspect() starts a fresh check;
asks the worker to dispose;
releases waiting readers with the bounded unavailable result instead of an endless wait.
The file-replacement barrier is unchanged. quiesce() and acceptRebuilt() still wait for every scanner's actual thread exit, including timed-out ones, before an index is replaced. The timer is injectable (checkTimeoutMs, scheduleTimeout) so the regression runs on a fake schedule with no sleeps.
RebuildWorkerJob.build() keeps its current contract. A full rebuild of a large ledger can legitimately run past 30 s, so this bound does not apply to that path.
structure/dashboard-and-usage.md documents the bound and the barrier rule.
Verification
Windows, Bun 1.4.4, base dev8063be23e. Tests ran one process at a time.
RED on unmodified code: the new case in tests/usage/request-history-integrity.test.ts (a createJob whose check() never settles or exits) fails, 20 pass / 1 fail.
Not run locally: the full suite, because this Windows host is shared with other lanes and allows one focused test process at a time. Full coverage is left to PR CI.

Bug Fixes
Integrity checks now time out after 30 seconds and report an operational failure if they do not complete.
Waiting readers are released when a check times out instead of remaining blocked until the scanner exits.
Rebuilds and shutdowns wait for active scanners to exit before proceeding, including scanners that timed out.

Walkthrough
Integrity checks now have a configurable timeout, defaulting to 30 seconds. A timed-out check records an operational failure and releases waiting readers, while its worker-exit barrier remains in place until the worker exits. Inspection, rebuild acceptance, and quiescence account for checks that are still draining.

Summary
Timeout configuration and outcome src/routing/history/integrity.ts
The monitor adds a 30-second default timeout and configurable timeout scheduling. On timeout, it records a failure when the generation is current, active, and not corrupt; disposes the job; and releases the caller-facing promise.

Inspection, rebuild acceptance, and quiescence account for draining checks. Tests cover timeout behavior, late verdicts, shared readers, and waiting for worker exit. The usage documentation describes the timeout and drain contract.

Request-history integrity checks now time out after 30 seconds, which fixes the indefinite reader wait. If the scanner is stuck in native I/O, the retry after each cooldown can leave extra worker threads running. This is an edge case, but a cap on stuck workers should be added before merge, or the risk explicitly accepted.

Full details: Linked Issues check
Explanation
Issue #7984 requires four coding behaviors. Each is implemented in src/routing/history/integrity.ts at the reviewed head.
Bounded check: REQUEST_HISTORY_INTEGRITY_CHECK_TIMEOUT_MS = 30_000 is exported. RequestHistoryIntegrityMonitor.run() starts a timer through scheduleTimeout and races it against the job check, cancellation, and the timeout. The check now ends within the bound.
Operational failure and cooldown: on a timeout outcome, run() calls recordFailure() when the generation is current, the check is not cancelled, and the generation is not corrupt. recordFailure() sets nextCheckAt to now + failureCooldownMs. A later inspect() after the cooldown starts a new check through start(). A late verdict cannot change state because the race already settled.
Worker disposal: the timeout branch calls job.dispose(). RebuildWorkerJob.dispose() (src/routing/history/rebuild-job.ts) calls worker.terminate(), so a silent worker is stopped.
Bounded reader result: release() resolves the caller-facing promise. inspect() then returns { state: "unavailable" } with the recorded failure error when no verified verdict exists.
File-replacement barrier: each check is added to the draining set. Its barrier is the run() promise, which awaits job.afterExit() before the check leaves draining. quiesce() returns Promise.all over draining barriers. acceptRebuilt() throws while draining.size is non-zero. A timed-out worker therefore still blocks replacement until its real exit callback runs. This matches the issue requirement.
Non-goal: RebuildWorkerJob.build() and send() are unchanged, as the issue and PR allow.
Limits: the test file diff could not be read (changed blobs unavailable in the checkout). The regression test content comes from the supplied PR summary and author-reported RED/GREEN results. I did not verify that the test asserts dispose() directly. The issue describes that assertion only as a sketch, so its absence does not fail the implementation requirement.
Note on the PR description: it says acceptRebuilt() "waits" for thread exit. In the code, acceptRebuilt() throws while checks drain. The caller must await quiesce() first. The code behavior is unchanged from base.
Full details: Out of Scope Changes check
Explanation
All changes trace to the #7984 objective or to the change that supports it.
New constant and injectable options (checkTimeoutMs, scheduleTimeout): needed for the bound and the fake-schedule regression test.
run() timeout branch: implements the bound, failure recording, dispose, and early release.
draining Set, check.barrier, and the updated quiesce() and acceptRebuilt() guards: required so that timed-out workers, which may still be alive after release, still hold the file-replacement barrier. Replacing active?.promise with a barrier over all draining checks follows from this.
inspect() pending branch for checks from older generations: needed so that a new generation does not replace an index while an old scanner still runs.
Shortened state-type comment: the old text said completion proves the scanner exited. Timed-out checks now release before exit, so that claim is no longer true. The change is required for accuracy.
structure/dashboard-and-usage.md (+1/-1): documents the bound and barrier. The documentation diff could not be read, so this is taken from the summary.
Regression tests in tests/usage/request-history-integrity.test.ts: supporting coverage for the new behavior.
I found no unrelated cleanup, refactoring, or behavior change in the reviewed evidence.

After a timeout, active becomes null, and inspect() ignores same-generation entries in draining. The 30-second cooldown can therefore start another check while the previous worker is still alive.
RebuildWorkerJob.dispose() calls worker.terminate(), but waits only 5 seconds. The worker performs synchronous PRAGMA quick_check, and the implementation allows native I/O to outlive that disposal bound. Repeated scans that remain alive across cooldowns can therefore accumulate without a source-level capacity limit.
Add a capacity limit for timed-out workers that have not actually exited. Return unavailable while that capacity is full. Track actual worker exit rather than only draining, because afterExit() schedules its cleanup callback after an overrun but resolves its own promise before that callback runs. Document that the fresh-after-cooldown retry is subject to this capacity limit.

A fresh exact-head confirmation on d35748c6a returned PASS: a late healthy or corrupt verdict after the timeout cannot change the generation's state, and the replacement barrier still waits for the worker's actual exit.
