Switching on/off the mods, resource-pack problem

Describe the bug

when I try to disable mod I got this message:
An error occurred
Error interacting with database: error returned from database: (code: 2067) UNIQUE constraint failed: instance_files.instance_id, instance_files.relative_path
and then I clicked refresh mod got disabled but this error message appearing every time I click enable/disable
