Autocomplete repeatedly throws macOS permission error: unable to query focused text context; Not authorised to send Apple events to System Events (-1743). Expected: one clear actionable permission prompt and graceful fallback. Actual: continuous repeated error spam. Tasks: detect permission before querying, throttle duplicate errors, show one recovery CTA, suppress repeated toasts, add denied-permission regression test.

    Resolved by #460 + #495 — the repeated Apple Events -1743 errors were caused by orphaned unified_helper_bin processes continuing to query accessibility APIs after the app exited. With graceful shutdown (#460) and proper SIGTERM signaling (#495), the helper is cleanly terminated and no longer spams permission errors.
