feat(sdk): tier public surfaces and enforce transport contracts - #707

Deliver capability-scoped SDK entrypoints, semantic API snapshots, import and CLI transport budgets, extension runtime coordinates and actionable activation diagnostics. Unify automatic harness author attribution, advisory duplicate governance, bare boolean mutation flags, cross-platform Node 22 index behavior, and package-first verification. Record all implementation, tests, documentation, graph lineage, and append-only author dispositions through pm.
