Partial implementation of #10, stacked on PR #119.

Dependency / live alignment

Current live prerequisite is exact PR #119 head ba16263f58e8c21df909e881b390cc094baa6642. Current #121 head is 6154bebb451e2a166b0ffb2a7ac913fd0d8f8d6c.

This head merges the current prerequisite without rewriting history. Fresh comparison reports ahead 16 / behind 0, with merge base exactly the live prerequisite. The delta remains limited to four #121-owned paths: CHANGELOG.md, crates/originweave-evidence/src/lib.rs, crates/originweave-evidence/src/sensitive_break_glass.rs, and crates/originweave-evidence/tests/sensitive_break_glass_evidence.rs.

Keep Draft while #119/#90 and their prerequisite sensitive-data/model-policy stack remain active. No prerequisite or predecessor check, review, status, coverage, mergeability, or synthetic-merge evidence transfers.

Implemented contract

SensitiveBreakGlassEvidence is an immutable credential-free receipt for one actual exceptional disclosure. Construction validates exact request/decision authority, beneficiary actor binding, reason, approval cardinality, bounded authority interval, decision/disclosure ordering, heightened-monitoring correlation, mandatory review metadata, and retention extending beyond review. Malformed identifiers, field sets, actor or approval mismatch, invalid timing, non-disclosure outcomes, and insufficient retention fail closed.

On exact head 6154bebb451e2a166b0ffb2a7ac913fd0d8f8d6c against exact live prerequisite ba16263f58e8c21df909e881b390cc094baa6642:
