fix(responses): notice a Codex WebSocket that dies after the response started - #7725
Once the first Responses event committed the 200, the Codex WebSocket exchange stopped pinging and cleared its silence timer, so nothing in the proxy could tell a quiet turn from a dead socket. A VPN or tunnel restart, a Wi-Fi rejoin or an expired NAT mapping drops the path without a FIN or RST, the committed stream goes quiet, and the turn waits out codex-rs's 300 s stream_idle_timeout before Codex retries it with Reconnecting... 1/5 (stream disconnected before completion: idle timeout waiting for SSE).
Field data from one install on 2.92.0 whose local tunnel restarts every few minutes: 24 attempts ended as a client cancel after 301 to 315 s with 4 or 5 upstream frames, 2 or 3 relayed events, pings=0 pongs=0 and no close code. Each began 2 to 19 s before a tunnel restart, and the retry of the same request finished in 5 to 38 s. Of 1,558 requests in the same window that no restart overlapped, 3 stalled.
The pinger now runs for the life of the exchange. One probe goes out with the create frame, because the pre-output refusal hold ([Bug]: Pooled native Codex requests do not retry or fail over on upstream server_is_overloaded / upstream_server_error before output #6969) can keep the commit back for up to 10 s and a path can die inside that window; without the probe the stream would have no pong to judge by. After the commit, a peer that has answered a ping on the exchange and then sends no frame and no pong for two consecutive ping intervals (CODEX_WS_COMMITTED_SILENT_INTERVALS) ends the stream as a body error, which the relay closes with its upstream_resetresponse.failed tail. codex-rs retries that after about 200 ms. A dead path is named 30 to 45 s after its last sign of life, or sooner when a ping draws a reset from whatever replaced the path. Each tick compares a count of inbound frames and pongs, so a long stream's per-frame path gains no timer work.
Unchanged: the proxy still makes no claim about how long a model may think. A peer that keeps answering pings is never cut however long the model is silent, a peer that never answered a ping keeps the client's own deadline, and the 90 s prelude bound and its 504 are as they were. Transports without the metadata channel (the opt-in api.openai.com socket) still commit at send and are not pinged. Nothing is replayed.
Behaviour to know about: every ChatGPT WebSocket exchange now sends at least one ping, so the stage record's pings and pongs read 1 or more where they used to read 0, and the prelude liveness test in ws-upstream.test.ts counts one more ping.
Not addressed: a socket that dies before any Responses event still settles through the existing reset path. When the network stays down longer than its one replacement send, that ends as 429 upstream_reset_replay_refused, which Codex does not retry.
Docs: the WebSocket prelude contract in structure/transports/responses-wire-shapes.md, and a triage section for idle timeout waiting for SSE in reference/codex-retry-diagnostics.md.
Verification
With Bun 1.4.4, on dev at 50afcec56 plus this diff unless noted. The branch was then rebased onto 8ebe46b20 without conflicts. The head is b745b5912, which differs from the reviewed d3e56e750 only in the triage docs and two structure lines.
Driven red first. In tests/responses/ws-committed-liveness.test.ts, the dead-socket case never settled on dev. Each of the other four cases fails against a deliberately broken build: pongs not counted as life (a live but silent peer is cut), frames not counted (a streaming peer that stopped answering pings is cut), no pong-evidence gate (a peer that never answered is cut), and no probe with the create frame (a path that dies inside the 10 s hold is missed).
The new file: 5 pass. With every tests/responses/ws-*.test.ts and codex-ws-*.test.ts file, the steering files, reasoning-archive-ws, reserve-dispatch-ws, responses-hosted-send-ws and tests/server/relay-eager.test.ts: 651 pass, 1 skip, 0 fail across 23 files. tests/test-layout.test.ts, tests/test-layout-tooling.test.ts and tests/ci-workflows/file-size-ratchet.test.ts: 38 pass. The 13 tests/ci-workflows/docs-*.test.ts files: 336 pass.
End to end with the real Codex client (codex-cli 0.164.0-alpha.11.1, default 300 s idle timeout and 5 retries), through the proxy, a TCP relay that freezes the first connection 1.5 s after the upstream's first bytes (no FIN, no RST, no bytes either way) and a local stand-in for the ChatGPT endpoint that answers pings: 2.92.0 took 311 s and printed idle timeout waiting for SSE; this branch took 41 s and printed Reconnecting... 1/5 (stream disconnected before completion: Upstream stream terminated unexpectedly: codex websocket stopped answering after the response started [cause=after-response-started ... pings=2 pongs=1]). Both finished on the retry.
In use on the same 2.92.0 install with this diff applied: 30 WebSocket requests, all 30 with a probe ping and 29 with a pong (the last died before any frame). Four paths died; all four ended within 20 to 34 s, and codex-rs logged retrying sampling request (1/5 in 186ms) for the committed ones. No attempt reached the 300 s timeout.
Full suite on Linux (2 vCPU): 52,574 pass, 2 fail, 177 skip. Both failures are environmental: Codex CLI selection inspection boundary > plan identity binds the bytes a real npm-global install placed hit the 5 s test timeout under full-suite load and passes alone with and without this change, and connected-client runtime probe scope > a rejected preferred runtime falls back without rewriting the saved selection fails the same way on untouched dev at 50afcec56. An earlier pass also timed out large output arrays stay ordered below the replay byte limit in ws-native-steering.test.ts; alone it takes 2.3 to 3.6 s with this change and 2.9 to 4.0 s without it on that machine.
On the head d3e56e750: the WebSocket set above plus the layout and size gates, 690 pass, 1 skip, 0 fail across 26 files.
On the head b745b5912 (docs and structure only since d3e56e750): bun run structure:check passes, and bun run build in docs-site builds 593 pages with internal links checked.
CodeRabbit's finding is applied: the pinger stays scheduled when ping() throws, so a socket that throws instead of closing is still judged by its silence. The new case for it fails on the reviewed head and passes now.
bun run typecheck, bun run structure:check, bun run privacy:scan and git diff --check: passed on both the original base and the rebased head. In docs-site/, bun install --frozen-lockfile and bun run build: passed, 593 pages built and 84,890 internal links checked.

Bug Fixes
WebSocket response streams now detect connections that stop responding after a response begins, helping end stalled streams with an upstream connection error. Streams remain active when the connection continues to respond, even during periods without model output. A single unanswered ping does not end the stream.
Documentation
Added guidance for diagnosing idle-timeout and WebSocket connection issues, including relevant request-log signals and timeout behavior. The guidance explains how increasing the idle timeout affects detection.

Walkthrough
The Responses WebSocket exchange now monitors committed streams for liveness. It continues pinging after commitment and fails a previously responsive peer after two silent intervals. Tests and documentation cover the behavior and idle-timeout diagnostics.
The exchange sends an initial ping after dispatch and continues pinging after commitment. If a peer has previously ponged and then sends no frame or pong for two consecutive intervals, the exchange fails the stream. The wire-shape documentation describes this behavior.
Liveness validation and diagnostics tests/responses/ws-committed-liveness.test.ts, tests/responses/ws-upstream.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json, docs-site/src/content/docs/reference/codex-retry-diagnostics.md
Tests cover silent and responsive peers, pre-output holds, stream outcomes, and cleanup. The test layouts include the new suite. The diagnostics guide distinguishes upstream silence from tunnel resets and describes diagnostic evidence.

Explanation
The title "fix(responses): notice a Codex WebSocket that dies after the response started" matches the main change. The changeset adds post-commit liveness probing to codexWsExchange, a new CODEX_WS_COMMITTED_SILENT_INTERVALS constant, and tests that detect a peer which ponged earlier and then goes silent after the response has started. The title names the scope (responses), the transport (Codex WebSocket), and the behavior (detecting a silent socket after commit). The objectives and summaries agree with this reading. The title is 77 characters, which is within a reasonable length for a commit subject.

Reviewed d3e56e75055fd453a4dd87304e249d768de00e36 and the complete eight-file diff. Independent official Bun 1.4.4 run with matching frozen dependencies/private homes and cgroup caps:
bun test --isolate tests/responses/ws-committed-liveness.test.ts tests/responses/ws-upstream.test.ts -> 133 pass, 1 skip, 0 fail, 999 assertions. This covers the new fake-clock silent/pong/frame paths and existing exchange/terminal/upgrade/size controls; one older-runtime case is skipped. Fake socket/timer coverage is not proof of a real VPN/NAT outage or macOS Bun behavior.
The commit clears the old timer before starting the committed loop; cleanup retains the terminal/cancel ownership, and the failure settles the committed body instead of adding another proxy create send. Peers with no pong evidence preserve the previous idle behavior. No runtime correctness blocker found in these scoped paths.
Please narrow the diagnostic prose “usually means the network path died”: an idle-client report alone does not establish that frequency or root cause. Describe a possible silent-path failure, and qualify the 30–45s bound to a metadata-capable canonical peer that previously ponged and has no frame/pong activity; healthy pongs and never-ponging peers intentionally keep the client idle deadline.
I approved existing exact-head Cross-platform CI 40199076682 and Catalog async contracts 40199076671 after inspecting the diff and unchanged workflow/install surfaces. Leaving COMMENT pending their actual completion, exact-head readiness and the prose clarification. No full local suite, native client inference or Codex Security scan was run.
Addressed in 00eb7c6, docs and structure only (no code change since d3e56e7). The triage section names a silent path failure as one possible cause and says the report alone does not show which. The 30 to 45 s bound now applies only on the canonical ChatGPT Codex endpoint with the metadata channel, after the response starts, to a peer that answered a ping on that exchange and then sent no frame and no pong; the structure note treats that peer as a dead socket instead of calling it one. Ponging peers, never-ponging peers and exchanges without the metadata channel keep the client's idle deadline on purpose. structure:check passes and docs-site builds; the new head needs the CI approvals again.

The exchange stopped pinging once the first Responses event committed the
200, so a path that died without a FIN or RST kept the committed stream
silent until codex-rs's 300 s stream_idle_timeout. Keep the pinger for the
whole exchange, send one probe with the create frame, and end a committed
stream whose peer answered a ping and then sent no frame and no pong for
two intervals.
