feat(sdk): ship defect recurrence and boundary evidence contracts - #1043
Conversation
Add typed boundary fixture validation, deterministic recurrence indexing, risk reports, producer census, and a shared assurance action across SDK, CLI, and MCP surfaces. Enforce repository defect evidence with captured external samples, escape-class policy, negative controls, performance budgets, generated contracts, agent-efficient token ceilings, comprehensive documentation, and exact coverage. Close the completed PM lineage with rich historical relationships and retain follow-up work for exhaustive runtime producer execution.

📝 WalkthroughSummary by CodeRabbit
WalkthroughAdded defect recurrence governance across the SDK, CLI, release gates, boundary fixtures, recovery-reference census, runtime value schemas, policies, tests, documentation, and PM evidence records. Added the ChangesDefect recurrence governance
Estimated code review effort: 5 (Critical) | ~120 minutes Merge Risk: 🟠 High · up to This change adds a new risk action and release evidence enforcement, but the current head still has contract and runtime correctness problems: schema-validated callers may reject the action, malformed metadata can abort the gate, and malformed or deeply nested inputs can hang or cause excessive work. These issues can block or destabilize release-time analysis, so the PR is not safe to merge until the high-impact cases are fixed or explicitly accepted. Sequence Diagram(s)sequenceDiagram

In `@config/defect-recurrence-policy.json`:
- Around line 16-19: Update the checks configuration so the local entry
references a runnable command: either define quality:record-integrity in
package.json or replace that reference with an existing script, while preserving
the intended record-integrity validation.
In `@scripts/bench/defect-recurrence-index.mjs`:
- Around line 61-63: Extend the determinism result in the benchmark check to
rebuild the incremental update from an equivalent full index and compare that
fingerprint with incremental.index_fingerprint. Preserve the existing full-index
fingerprint comparison, and ensure the combined result is what the reporting and
ok logic use for incremental determinism.
In `@scripts/release/defect-evidence-gate.mjs`:
- Around line 67-73: Update policyReport and the JSON/text rendering paths to
evaluate and emit findings only for checks included in selectedChecks, including
the --boundary-only flow from resolveContext. Ensure unselected historical
reports are omitted or explicitly marked not evaluated so they cannot contribute
findings or gate failures.
In `@src/sdk/agent/refusal-reachability.ts`:
- Around line 237-260: Update the producer scan around propertyPattern and the
source iteration to parse each TypeScript file and inspect only executable
object-literal property assignments, excluding comments, string contents, and
type literals. Preserve the existing RECOVERY_REFERENCE_FIELD_CONTRACTS
classification, unknown_recovery_field detection, source ordering, and line
reporting while ensuring textual matches cannot contribute producers or make
report.ok true.
In `@src/sdk/governance/assurance-action.ts`:
- Around line 466-470: The assurance risk transport currently rebuilds the full
recurrence index on every request. Update the flow around
buildDefectRecurrenceIndex and analyzeDefectChangeRisk to reuse a cached index
keyed by policy_fingerprint, pass it as previous_index, and provide
changed_item_ids so incremental rebuilding occurs; preserve correct invalidation
when the policy or tracked items change.
- Around line 460-477: Limit the try/catch in the assurance risk flow to
parseDefectChangeRisk and analyzeDefectChangeRisk, leaving
createAssuranceWorkspaceContext outside so workspace I/O and parsing failures
propagate with their own errors. Preserve the existing PmCliError passthrough
and EXIT_CODE.USAGE mapping for request parsing and analysis failures.
- Around line 443-459: Update runRiskAction to reject any supplied input.limit
for the risk action with the established usage-error mechanism, rather than
silently ignoring it; preserve the existing risk request default and behavior
when limit is absent.
In `@src/sdk/governance/boundary-fixtures.ts`:
- Around line 202-218: Update evaluateBoundaryRecord so captured is 1 only when
a valid fixture exists and validateSample returns no findings; return captured:
0 for missing, invalid, unsafe, or failing samples while preserving the existing
findings and waived values. Keep BoundaryFixtureReport.captured_count’s
documented evidence-coverage meaning.
- Around line 28-29: Update the boundary-fixture loader used by
defect-evidence-gate to resolve fixture_path against repositoryRoot and reject
any resolved path outside repositoryRoot before reading it. Preserve valid
committed fixture paths, and ensure traversal values such as parent-directory
segments cannot escape the repository boundary.
In `@src/sdk/governance/defect-recurrence.ts`:
- Around line 358-369: Update fileReasons to compute
uniqueSorted(family.triggers.file_patterns ?? []) once before iterating over
files, then reuse that pattern list inside the per-file loop while preserving
the existing matching and reason ordering.
- Around line 328-335: Update pathMatches to reuse compiled glob regular
expressions instead of constructing a new RegExp for every comparison, using a
cache keyed by the pattern and keeping it bounded or scoped to a single index
build for long-lived sessions. Validate and reject patterns exceeding a defined
maximum length before compilation, while preserving the existing glob-matching
behavior.
- Around line 730-761: Complete structural validation in
parseDefectRecurrencePolicy and parseDefectChangeRiskRequest so untrusted values
match their declared types before private helpers run. Extend validatePolicy to
validate each family object, historical_item_ids, budget, checks, triggers, and
negative_control using the existing DefectChangeRiskInput validator, ensuring
negative controls select their own family. In parseDefectChangeRiskRequest,
validate change with that helper and enforce runtime numeric validation for
cursor and limit, while preserving field-specific usage errors.
- Around line 478-501: The canReuse condition in the index-building flow should
reuse a defined previous_index whenever its policy_fingerprint matches,
including when changed_item_ids is empty; retain filtering of changed item IDs
and preserve prior mappings for an empty items projection. Remove the redundant
itemsReused reset guarded by !canReuse.
In `@tests/unit/scripts/release/defect-evidence-gate.spec.ts`:
- Around line 146-154: Add an afterEach lifecycle hook for this test suite that
calls vi.restoreAllMocks(), and remove the assertion-dependent
processOutput.mockRestore() and processError.mockRestore() calls. Keep the
existing stdout and stderr spy assertions unchanged while ensuring all mocks are
restored even when an assertion fails.

In `@config/boundary-fixtures.json`:
- Around line 3-68: Align inventory_scope with the registered boundaries: either
add a Git boundary entry with its captured fixture or an expiring waiver, or
remove Git from inventory_scope. Update the boundaries registry while preserving
all existing boundary entries and metadata.

- Around line 46-70: Update the benchmark around changedItems and incremental to
construct the complete post-change corpus, rebuild its index, and require the
rebuilt index_fingerprint to match incremental.index_fingerprint in the

lines 24-64, add a deterministic incorrect incremental implementation and assert
the benchmark reports failure when it differs from the rebuilt updated corpus.

- Around line 9208-9213: Extend the PM recovery field-contract census
represented by PM_RECOVERY_REFERENCE_FIELDS to include next_best_command and
fallback_candidates, preserving the existing readonly tuple signature and field
ordering conventions.
In `@src/sdk/agent/refusal-reachability.ts`:
- Around line 334-338: Optimize the loop in the refusal-reachability census to
avoid repeated prefix slicing and newline splitting for every property. In the
per-source processing around scanSourcePropertyTokens, precompute line starts or
track line numbers once, and defer line computation until after field contract
or unknown-field filtering.
- Around line 334-385: Replace all localeCompare calls in the producer and
finding sort logic with a locale-independent code-unit comparator, covering
source paths, producer fields, finding subjects, and finding kinds. Preserve the
existing primary and tie-breaker ordering while ensuring identical input
produces the same census order across runtimes.
- Around line 307-323: Update scanSourcePropertyTokens to recognize only static
property members within object literals, rather than collecting every quoted or
identifier token encountered lexically; exclude destructuring aliases, labels,
regular-expression text, and template expressions so report.ok cannot be
satisfied by non-executable recovery producers, and add negative controls
covering those cases.
In `@src/sdk/governance/assurance-action.ts`:
- Around line 518-537: Replace the full serialized values used for item
fingerprints in the riskIndexCache with fixed-size digests, updating the
fingerprint creation and changedRiskItemIds comparison so change detection
remains equivalent. Apply the same digest-based treatment to policy_serialized
if policy size can be large, while preserving the existing cache reuse behavior
in the surrounding risk-index flow.
In `@src/sdk/governance/boundary-fixtures.ts`:
- Around line 220-245: Update the waiver result in the boundary evaluation flow
so `waived` is 1 only when `validWaiver` is true and the waiver has not expired;
invalid or expired waivers must report zero while preserving their findings. Use
`BoundaryFixtureReport.waived_count` and the nearby waiver logic as the scope,
unless the intended contract is instead to count all inventoried waivers, in
which case revise its documented meaning accordingly.
- Around line 277-286: Update the boundary-validation loop around
registry.boundaries to reject null and non-object entries before accessing
boundary.id, adding an invalid_boundary finding and continuing validation for
each malformed entry. Preserve duplicate_boundary handling for valid object
entries.
In `@src/sdk/governance/defect-recurrence.ts`:
- Around line 800-805: Update the governed-item filtering around isDefectItem so
terminal defect items with missing or malformed completed_at are governed rather
than silently excluded, allowing DefectGateEvidenceReport to emit a finding and
keep the gate fail-closed. Preserve the epoch check for valid timestamps, and
require an explicit allowlist or recorded waiver before excluding historical
items without valid completion timestamps.
- Around line 462-481: Update itemSignals to filter item.tags to string values
before assigning tags, matching the package_names and error_codes handling; also
update isDefectItem to ignore non-string tags before calling toLowerCase,
preserving valid string-tag behavior.
- Around line 286-290: Replace locale-dependent ordering with a deterministic
total code-unit comparator in uniqueSorted and the family/item ordering paths
around the referenced ordering logic. Ensure distinct valid strings, including
canonically equivalent forms, never compare equal and that policy_fingerprint
and index_fingerprint remain independent of runtime locale and input order;
leave presentation-only sorting unchanged.
In `@tests/unit/sdk/assurance-action.spec.ts`:
- Around line 735-747: Update the assurance-action test around
runAssuranceAction so the request signal incorporates createdId and its
membership changes after the update and deletion mutations. Assert the resulting
changed risk reason, result, or index_fingerprint rather than repeating the
unchanged risk_detected and total expectations.
- Around line 682-686: Update the test around the created item and
assurance-transport request to include an item trigger and pass createdId
through change.item_ids, then delete that item and assert the resulting report
changes from risk_detected true and total 1. Use the existing createdId from the
create response and preserve the current file-based trigger coverage.

Acknowledged as a provider limitation, not approval. The requested final exact-head review could not start because the provider rate limit remained active. Merge confidence therefore relies on the completed actionable bot rounds, resolved review threads, exact-head hosted gates, repository-wide local verification, and manual packed-artifact testing. |
