Summary

A deep audit + remediation pass over the pm CLI + SDK + ecosystem (ADR pm-96wm). Four specialized read-only sub-agents audited code-quality/perf, SDK/extensions, agent-UX/token-efficiency, and docs/CI; the ecosystem was verified end-to-end in a temp sandbox + project smoke harnesses. Findings were recorded centrally in pm (deduped against the open backlog) to avoid concurrent-write history drift. This PR ships the focused, low-risk batch; larger findings are tracked (see below).

Philosophy: project management = context management. Every fix here either stops an agent from dead-ending or trims wasted context.

Never-block (the most-emphasized goal)

  • pm close / close-many without a reason returned a generic error that named the internal governance.require_close_reason knob but never told the agent how to recover — the single most common agent dead-end (default-on in every preset). Now returns actionable guidance (examples, next steps, how to disable). pm-g799
  • --path / PM_PATH at a project root hard-failed tracker not initialized while cwd-mode discovers .agents/pm. resolvePmRoot now redirects an explicit project-root into its .agents/pm, keeping pm init --path and direct tracker roots verbatim. pm-ryik

MCP / contract coherence

  • serverInfo.version now reflects package.json (was hardcoded "1.0.0"). pm-2nvw
  • error envelope includes structuredContent.result = null so consumers read .result uniformly across success/error. pm-l40h
  • pm_health defaults to the compact summary projection (~22× smaller; full=true opts into the deep payload). pm-yjub
  • corrected MCP tool-count docs (18/21 → 22, +pm_copy; 3 → 4 subagents) and locked the claude-plugin smoke to the live tools/list count. pm-7tvx

CI / docs

  • pinned codeql.yml init/analyze to immutable SHAs + a contract-test assertion that codeql.yml has no unpinned actions. pm-ji5c
  • corrected the inaccurate PR CI-matrix description in CONTRIBUTING.md. pm-t73k

Cleanup

  • removed dead code (a dead IIFE in runtime-schema.ts, unused NESTED_SETTING_KEYS and _backgroundRefreshInternals) and deduped four package.json version readers into resolvePmCliVersion (drops unused fs/path from the main.ts hot path). pm-wrqk

Verification

  • pnpm typecheck ✓ · pnpm build ✓
  • 2559 tests @ 100% coverage (stmts/branches/funcs/lines) ✓ (+5 new tests)
  • static-quality, contracts, docs/skills, security gates ✓
  • claude-plugin smoke ✓ (22 tools) · package-first dogfood ✓
  • both never-block fixes verified end-to-end in a temp sandbox

Deferred findings (tracked, not in this PR)

Recorded for follow-up under the existing capability epics (kept out to keep this PR low-risk):

  • Perf: pm-2bn5 settings read cache — the biggest remaining per-command lever; deferred because safe memoization must key on settings.json and schema/*.json mtimes (schema files change independently) and stay correct for the long-lived MCP server.
  • Agent-UX: pm-vhx6 compact-mode trailer trimming · pm-6l17 --tags never-block alias · pm-jozc MCP top-level filter-key hoisting · pm-q0kr slim guided-error boilerplate.
  • SDK/ext: pm-oll8 field-type validation w/ did-you-mean · pm-l0jd manifest JSON Schema + getWorkspaceContracts memo + FlagDefinition unify; enriched pm-ltbr/pm-1js9/pm-k1e4.
  • Code-quality: pm-e7dn runUpdate table-driven refactor · pm-ul02 remove as unknown as casts.

Deep audit + remediation pass (ADR pm-96wm). Four specialized read-only
sub-agents audited code-quality/perf, SDK/extensions, agent-UX, and docs/CI;
findings were recorded centrally in pm (deduped against the open backlog).
This ships the focused, low-risk batch.

Never-block (agents must never dead-end on the common path):
- pm close / close-many without a reason returned a generic error that named
the internal governance knob but not the fix; now returns actionable
guidance (examples, next steps, how to disable
  governance.require_close_reason). [pm-g799]
- --path / PM_PATH pointing at a project root hard-failed "tracker not
initialized" while cwd-mode discovers .agents/pm; resolvePmRoot now redirects
an explicit project-root into its .agents/pm, keeping pm init --path and
  direct tracker roots verbatim. [pm-ryik]

MCP / contract coherence:
- serverInfo.version now reflects package.json (was hardcoded "1.0.0"). [pm-2nvw]
- error envelope includes structuredContent.result=null so consumers read
.result uniformly across success and error. [pm-l40h]
- pm_health defaults to the compact summary projection (~22x smaller; full=true

- corrected MCP tool-count docs (18/21 -> 22, +pm_copy; 3 -> 4 subagents) and
locked the claude-plugin smoke to the live tools/list count. [pm-7tvx]

- pinned codeql.yml init/analyze to immutable SHAs + a contract-test assertion
that codeql.yml has no unpinned actions. [pm-ji5c]
- corrected the inaccurate PR CI-matrix description in CONTRIBUTING.md. [pm-t73k]

Cleanup:
- removed dead code (runtime-schema IIFE, NESTED_SETTING_KEYS,
_backgroundRefreshInternals) and deduped four package.json version readers
into resolvePmCliVersion (also drops unused fs/path imports from the main.ts
  hot path). [pm-wrqk]

Verified: typecheck, build, 2559 tests @ 100% coverage, static/contracts/docs/
security gates, and the claude-plugin smoke (22 tools). Deferred findings are
tracked under pm-mpbb/pm-5k4v/pm-ugqx/pm-o2kc (incl. the settings-read cache
perf lever pm-2bn5).

Changes

Production Code & Integration Changes

Layer / File(s) Summary
Centralized PM CLI version resolution
src/core/packages/root.ts, src/cli/main.ts, src/cli/commands/upgrade.ts, src/core/sentry/instrument.ts
New resolvePmCliVersion helper safely reads package.json and returns validated version or undefined; replaces inline file-read logic across CLI entry point, upgrade command, and Sentry instrumentation to ensure consistent version resolution.
MCP server enhancements: version, health, error envelope
src/mcp/server.ts, tests/integration/mcp-handshake.spec.ts
MCP server now derives serverInfo.version from real package version (not hardcoded 1.0.0); pm_health defaults to compact "summary" projection when no options provided; error responses consistently include structuredContent.result: null alongside error/code fields. Integration tests validate version matches numeric pattern, health projection modes, and error envelope structure.
PM root path resolution with nested tracker detection
src/core/store/paths.ts, tests/unit/store-paths.spec.ts
resolvePmRoot now detects explicit --path/PM_PATH pointing to project root and conditionally redirects into nested .agents/pm tracker directory when present; preserves back-compat for explicit tracker roots and existing behavior. Unit tests cover redirection and preservation cases.
Close command structured error details
src/cli/commands/close.ts, src/cli/commands/close-many.ts, tests/unit/close-command.spec.ts
Replace generic missing_required_argument errors with close_reason_required code and structured required/why/examples/nextSteps fields for governance-required close reasons; improves CLI error recovery guidance. Unit test captures error details and validates guidance fields.
Code cleanup
src/core/search/background-refresh.ts, src/core/config/nested-settings.ts, src/core/schema/runtime-schema.ts
Removed _backgroundRefreshInternals test hook, NESTED_SETTING_KEYS exported constant, and simplified variable initialization in loadRuntimeSchemaFromOptionalFiles.
MCP tool count verification in smoke test
scripts/smoke-claude-plugin.mjs
Updated required MCP tool list to include pm_copy; added assertion that live tools/list count exactly matches required array length, failing with guidance if tool surface diverges.
CI workflow security and contract tests
.github/workflows/codeql.yml, tests/integration/ci-workflow-contract.spec.ts
Pinned github/codeql-action/init and github/codeql-action/analyze from mutable @v3 tags to immutable commit SHAs; added workflow contract test that validates CodeQL action SHA-pinning and scans for unpinned action references.

They are never manually edited. Committing these files alongside the work they track is the established convention in this repo (see PRs `#36`–#39). Learning: In `unbraind/pm-cli`, the semantic/hybrid search fallback-on-any-error behavior in `src/cli/commands/search.ts` is intentional. The product requirement is that `pm search` must never block an agent. Every degradation emits a machine-readable `search_<mode>_fallback:<reason>:using_keyword_mode` warning (reason ∈ `timeout`|`connection`|`error`) returned to the caller and captured by telemetry. Sentry was explicitly rejected for this path because it is lazily initialized only in error paths and would impose async init latency on the hot (successful) search path.

Learning: In the pm-cli project, `history-redact`, `restore`, and `history-repair` all follow the same intentional read-compute-lock-write pattern: read history + item document outside the lock → compute the rewritten content → acquire the per-item lock → atomic write with rollback. Tightening the read inside the lock is acknowledged as a future hardening item but is intentionally deferred so the locking model stays consistent across all three sibling commands.

⚠️ Potential issue | 🟡 Minor | ⚡ Quick win

Avoid a future false-negative on the version assertion.

Line 71 will fail once the real package version legitimately becomes "1.0.0", even when wiring is correct. Assert against the repository package version instead of asserting inequality to a historical constant.

Review loop status:

  • CodeRabbit: reviewed, 0 actionable code findings. The one soft flag was Docstring Coverage 5.88% < 80% — I evaluated it and am intentionally not chasing the threshold: src/core/store/paths.ts and src/cli/commands/close.ts contain zero existing JSDoc and rely on inline comments, so adding /** */ blocks to only the touched functions would break the established file style (and the count is dominated by test callbacks). The changed production functions already carry clear inline rationale, and there is no docstring gate in CI. resolvePmCliVersion (the one genuinely new public helper) does have a full JSDoc.
  • Gemini: daily quota exhausted (could not review).
  • Codex: connector not enabled for this repo (could not review).
