Summary

This broad SDK and CLI delivery makes workspace integrity self-describing and repairable through shared public contracts:

  • preserve history hash epochs during audit, reanchor, and repair, with explicit before and after receipts
  • normalize invalid provenance metadata through SDK, CLI, and MCP surfaces without exposing raw identities
  • census extension-host package versions across npm, pnpm, and alternate layouts and make mismatches health-gate failures
  • return structured health findings with stable codes, severity, remediation, and compact failure causes, including deciding findings beyond bounded warning samples
  • reject malformed assignment-style linked artifacts atomically while preserving unmistakable paths and URLs, including values containing equals signs
  • keep packed npm and npx smoke execution reliable on fresh runners with dependency resolution and a 120-second deadline on every subprocess
  • preserve explicit token ceilings for extension-contributed commands across extension-absent and CI-installed topologies
  • refresh generated contracts, token baselines, documentation, and the pm-changelog output

PM lineage

Verification

  • node scripts/run-tests.mjs coverage — 500 files, 7,337 tests, exact 100/100/100/100
  • pnpm typecheck
  • repository static-quality assurance and direct docstring gate — 100% module, exported-symbol, and exported-member coverage
  • SDK import-cost and CLI transport-floor gates with unchanged ratchets
  • token-surface unit contracts — 12/12; context-quality assurance — 4/4 blocking assertions
  • real token ratchet — 79 active surfaces without search-advanced and 81 with the CI extension topology
  • pnpm smoke:npx — fresh packed tarball, version 2026.8.14, 13 extension packages
  • linked PM verification — all commands passed across every actively worked item
  • manual temporary-directory acceptance: npx and bunx install/version, initialize/create, malformed parser rejection, provenance normalization, chain verification, and health findings
  • pm health --check-only — 10/10 checks clean
  • pm graph audit --output-budget unbounded — zero findings across 2,492 nodes and 12,272 edges
  • GitHub dependency, code-scanning, and secret-scanning alert inventories — zero open alerts

Review resolution

CodeRabbit findings were fixed or explicitly resolved on their original threads in febd5a79c; all seven threads are resolved. The installed-extension CI regression is fixed in ab2166457 with the bidirectional ratchet unchanged. The repository-native docstring gate supersedes the reviewer estimate with exact 100% measurements. Sourcery and the latest CodeRabbit request were rate-limited and are treated as unavailable evidence rather than approval. Fresh CodeRabbit and Greptile reviews are requested after every pushed review fix.

Deliver version-stable history repair, provenance normalization, extension-host version census, self-indexing health findings, and strict linked-artifact parsing through shared SDK and CLI contracts.

Refresh generated surfaces, token baselines, changelog evidence, and packed npm/bun smoke coverage while preserving exact 100% test coverage and sandbox-safe project tracking.

Warning

Review limit reached

@unbraind, you've reached your PR review limit, so we couldn't start this review.

Next review available in: 20 minutes

You've used all free OSS reviews for now. Wait for the free limit to reset to keep reviewing this public repository.

How can I continue?

After more reviews become available, a review can be triggered using the @coderabbitai review command as a PR comment. Alternatively, push new commits to this PR.

To avoid repeated limits, reduce automatic review volume by pausing incremental auto-reviews earlier, using label-based review opt-in, excluding WIP or generated PR titles, or requesting reviews manually when the PR is ready. If your team needs uninterrupted high-volume reviews, an organization admin can enable usage-based reviews.

How do review limits work?

CodeRabbit enforces per-developer PR review limits for each organization. Most developers receive the normal plan review availability.

For paid Pro and Pro+ PR reviews, CodeRabbit uses adaptive limits for sustained high-volume activity. When a developer's recent PR review activity reaches the 95th percentile or higher among CodeRabbit users, additional reviews become available more gradually as earlier reviews age out of the rolling window.

Changes

Governance and repair

Layer / File(s) Summary
History repair and provenance normalization
src/core/history/replay.ts, src/sdk/history-repair.ts, src/sdk/governance/provenance-health.ts, tests/unit/sdk/*history*, tests/unit/sdk/provenance-normalization-contract.spec.ts
History repair preserves or selects hash versions, removes invalid provenance observations, records aggregate receipts, and supports bulk provenance-invalid stream repair.
Health findings and extension version census
src/sdk/governance/health.ts, src/sdk/governance/extension-host-version.ts, tests/unit/sdk/extension-host-version-health.spec.ts, tests/unit/sdk/health-finding-contract.spec.ts
Health results now include structured findings and gate-failing warnings. Extension scans report duplicate pm-cli versions across dependency layouts.
CLI contracts and linked-artifact parsing
src/cli/register-mutation.ts, src/sdk/cli-contracts/*, src/sdk/linked-artifacts.ts, tests/unit/sdk/linked-artifact-assignment-contract.spec.ts, tests/fixtures/contracts/full.json
The CLI and SDK expose provenance normalization. Assignment-shaped linked-artifact values require declared keys, while bare paths and URLs remain unchanged.

Release validation

Layer / File(s) Summary
Release smoke and replication validation
scripts/smoke-npx-from-pack.mjs, scripts/release/surface-replication-sets.json, tests/unit/scripts/*
Packed-artifact smoke commands run with 120-second subprocess limits. Replication checks activate on relevant changed lines and retain negative controls.
Release documentation and tracker records
CHANGELOG.md, docs/COMMANDS.md, docs/SDK_CONTEXT_INTEGRITY.md, sdk/public-surface.json, .agents/pm/*, scripts/agent-token-surface-baseline.json
Release notes, command documentation, SDK surface metadata, token baselines, and PM records describe the implemented behavior and verification results.

Estimated code review effort: 5 (Critical) | ~120 minutes

Merge Risk: 🟡 Moderate · up to ab216

The PR improves workspace integrity and health reporting, but malformed linked-artifact input can still bypass required validation, and some failing health results may omit metadata needed for reliable remediation. These are concrete current-head contract risks that should be fixed or explicitly accepted before merge.

File-Level Changes

Change Details Files
Health results become self-indexing with structured findings, explicit failure causes, and extension-host pm-cli version census.
  • Extend HealthResult to include structured findings array and failed_because tokens and propagate them through brief/summary projections.
  • Add buildHealthFindings to map warnings to owning checks, stable codes, severities, and remediation/disposition, using remediation sources and resolveRemediation.
  • Integrate extension host pm-cli version scanning into the extension health check, including warnings and host_version_census details.
  • Adjust health remediation sources to incorporate policy, normalized item read, history, and directory hook warnings; attach new remediation entry for invalid provenance values.

History repair gains provenance normalization, hash epoch preservation, bulk targeting for invalid streams, and CLI/tooling contracts.
  • Add normalizeProvenance option to HistoryRepairCommandOptions and wire CLI flags, tool schemas, and parameter metadata, including docs and examples.
  • Implement provenance normalization helpers to strip bounded-domain-invalid observations from history entries with aggregate receipts and stream discovery utilities.
  • Update reanchorHistoryEntries and resolveHistoryRepairItemHashVersion to retain or select item_hash_version epochs based on chain verification and explicit versions.
  • Extend history-repair results to report before/after hash versions, version disposition, and privacy-safe provenance_normalization receipts, plus new reconciliation warnings.
  • Teach runHistoryRepairAll to target both drifted and provenance-invalid streams and report provenance_invalid_streams counts.

Linked artifact parsing is tightened to treat any assignment-like input as structured and reject unknown keys, preserving bare-path compatibility only for non-assignment values.
  • Change looksLikeStructuredPathEntry to consider any raw containing '=' as structured, enforcing the path/scope/note allowlist.
  • Add tests ensuring assignment-like values are parsed via structured path contracts, bare paths/URLs remain compatible, and file/doc entries are transport-neutral.

Packed npm/npx smoke tests are hardened with offline local-tarball flows, bounded subprocess timeouts, and updated mocks.
  • Wrap smoke commands with a shared runSmokeCommand that enforces a 120s timeout and use it for npm pack, npx, and npm exec/install invocations.
  • Force --offline for npx and npm exec/install when operating on local tarballs, including bare package smoke and pm-cli alias tests.
  • Update test harness to accept timeout options in execFileSync mocks and verify offline flags and timeouts across calls.
  • Adjust smoke tests to detect file:-spec arguments with args.some rather than positional assumptions.
scripts/smoke-npx-from-pack.mjs
tests/unit/scripts/smoke-npx-from-pack.spec.ts
Release/replication and token gates, documentation, and changelog are updated to reflect new workspace-integrity contracts and timing.
  • Extend SDK context and command docs with provenance normalization, hash epoch preservation semantics, health findings, failed_because, and extension host version census.
  • Refine surface replication gate tests to consider flag-contracts changes and describe activation in terms of shared contracts.
  • Increase agent-task token gate timeout and refresh baselines and public surface artifacts for new governance exports and contracts.
  • Document fixes in CHANGELOG for provenance normalization, health findings severity, extension host skew detection, history-repair epoch stability, and linked-artifact parsing parity.

Keep deciding health findings visible after bounded projection, distinguish mixed writer epochs from consistent broken streams, and scan provenance incrementally without sacrificing complete repair discovery.

Preserve equals-bearing paths and URLs under strict linked-artifact assignment parsing, allow packed smoke dependencies to resolve on fresh runners, and keep every child process bounded.

Expand negative controls, stabilize real-worktree test deadlines, refresh generated changelog state, and record linked PM verification and lifecycle evidence.

CodeRabbit summary reviewed against febd5a7. All seven actionable findings have specific inline responses and regression controls. The repository-native docstring gate also confirms 100% module (523/523), exported symbol (4014/4014), and exported member (7328/7328) coverage, so the estimated 30.43% figure is not applicable to this repository contract.

Action guidance acknowledged. Findings were handled individually on their original threads so each decision, test, and intentional completeness tradeoff remains attached to the relevant code.

Sourcery did not produce an actionable review because the service was rate-limited. This is recorded as an unavailable review signal rather than approval; the fresh head is being retriggered for another terminal attempt.

CodSpeed result reviewed. Benchmark checks were green on the reviewed head; the unchanged local CLI transport-floor gate also passed in isolation after one transient 31 ms host timing outlier.

Codecov result reviewed. The updated head has 7,337 tests and exact 53,330/53,330 statements, 39,351/39,351 branches, 10,956/10,956 functions, and 51,499/51,499 lines.

This review is non-terminal evidence only: Sourcery reported a rate-limit condition and supplied no findings. It is not treated as approval, and a new review attempt will be requested for febd5a7.

Regenerate the bidirectional agent token-surface baseline under the same search-advanced topology used by CI so search-advanced and reindex retain explicit ceilings.

Verify both extension-absent and extension-present command surfaces without changing required-command semantics or headroom, and record the canonical PM regression lifecycle and generated changelog evidence.

Anchor linked-artifact URL classification so unknown assignment keys cannot bypass the shared SDK allowlist while genuine path and query equals remain compatible.

Give external extension-host copies deterministic opaque identities without adding SDK startup cost, and exercise nullable legacy provenance during real normalization verification.

Record exact review evidence in the canonical PM items and regenerate the tracker-derived changelog.

Learning: In `unbraind/pm-cli`, exact-head review evidence must not be required inside a PM history entry or derived task projection that is part of that same Git commit: appending via the PM CLI changes the append-only hash-chained history, synchronized projection, and commit head. Use the pull request’s exact-head hosted-check inventory as stable review evidence, and treat implementation commit references in PM records as traceability rather than self-attestation.

Learning: For GitHub pull request `#718` in unbraind/pm-cli, CodeRabbit quota/availability messages are operational notifications only and must not be interpreted as a substantive code-review finding or approval. Exact-head CI and independent validation artifacts are the relevant evidence.

Learning: For pull request `#735` in unbraind/pm-cli, a CodeRabbit full-review command that returns a fair-usage-limit notice is provider unavailability only. It must not be represented as substantive review evidence, a findings-free result, or human approval.

Learning: In unbraind/pm-cli `.agents/pm/` append-only PM evidence records, delivery evidence may cite an immutable implementation commit while mutable exact-head review and merge proof is maintained on the linked GitHub pull request. Do not require the PM record to embed the current PR head, because appending that record creates a new commit and makes such a claim self-referential and immediately stale.

Learning: In `unbraind/pm-cli`, the final merge decision uses the pull request hosted exact-head check inventory. PM history records retain implementation chronology and traceability only; they are not independent merge-validation evidence.
